ON-THE-JOB TRAINEE WAIVER

This waiver is in connection with the On-the-Job training program of

______________________________________ has requested and has been accepted and
confirmed by TOYOTA SAN PABLO, LAGUNA INC.

The student- trainee acknowledges that permission granted by TOYOTA SAN PABLO,
LAGUNA INC. is made under the condition that he/she accepts and agrees to and that the
company will not assume any responsibility for whatever injury or accident which may happen
to him/her within or outside premises during the period of the On-the-Job training program.

It is understood that there is no employee-employer relationship between the company and

the student trainee.

This waiver shall take effect during the duration of the On-the-Job training program, scheduled
