1. OBJECTIVE

To ensure the management, erection and installation work on site are safe,
correctly, and comply the contract specification.

2. PURPOSE

The purpose of this method of statement is to establish a work sequence on how


to erection and installation of steel works will be implemented. The statement
includes work methodology and sequence of activities all in accordance with the
contract specification for structural steelwork.
3. SCOPE OF WORK

The following works, define the activities which will be carried out for implementation the
erection and installation of steel work for according to the STP-5 Project:-

i) Transportation of Fabricated Portal Frame


ii) Unloading, Arrangement, Storage and Protect Materials
iii) Installation of Steel Columns
iv) Erection of Portal/Beam Frames

6.4 Head of Execution

6.4.1 Vests the overall responsibility for construction work and site work activities of the project.
6.4.2 Planning arrangements of resources of project.
6.4.3 Responsible for the overall SHE Management of the project.
6.4.4 Monitoring of overall progress and achievement as per the target of project.
6.5 Construction Manager
6.5.1 Construction manager signaling is responsible for all signaling activities of the project.
6.5.2 Construction manager Telecom is responsible for all telecom activities of the project.
6.5.3 Construction manager civil is responsible for all civil works of the project.
6.6 Site Manager

6.6.1 Vests the overall responsibility for construction work and site work activities at package level.
6.6.2 Monitoring the Program vs. Progress and achievement as per the target.
6.6.3 Plan and control manpower, equipment and materials according to the construction schedule at
package level.
6.6.4 Responsible for the overall construction quality of work at package level.
6.6.5 Responsible for all interfaces both internally and externally.

6.7.3 Liaison between main office and site execution team.

6.7.4 Liaison with internal departments like planning cell, Quality control cell, Plant & Machinery
department, Safety Department and stores for effective progress.

6.7.5 Submission of the requirement of materials & other resources for the next week/month to the
site Manager.

6.7.6 Documentation of all process related reports as per the ISO requirement, technical
specifications and engineer’s instructions, Contract review with sub-contractor.

6.7.7 Checking and control material, Equipment and manpower.

6.7.8 Ensure that all workmen engaged under him are selected through the screening system & have
undergone site SHE induction before assigning any task at a construction site.

6.7.9 Ensuring compliance of basic SHE rules and applicable specification as per SHE plan.

6.7.10 Ensuring SHE inspection has been carried out at a construction site and taking Prompt action of
site inspection and hazard findings.

6.7.11 Comply all the points identified in SHE inspection reports.

6.7.12 Ensure hazard/aspects identification and Risk/Impact assessment is done for all the critical job
under him.

6.7.13 To investigate all fatal accident, occupation diseases contracted and reportable dangerous
occurrences.

6.7.14 To promote and ensure the working of site SHE Committees at regular intervals, to act as an
advisor to such committees.

6.7.15 To advise purchasing and ensuring the quality of PPEs conforming to national standards.

6.7.16 To organize, in association with concerned departments, campaigns, competition, SHE


awareness days and other activities which will be developed and maintain the interest of
workers in establishing and maintaining safe condition of work and procedures.

6.7.17 To frame safe rules and safe working practices with consultation with project manager and
SAPO.

6.8 Site Engineer

6.8.1 This method statement, HIRA Document, and structure design drawing must be communicated
to all responsible people on site by the responsible person directing the work & must be
available at the site.

6.8.2 Ensure hazard/aspect identification and Risk/Impact assessment is done for all the critical job
under him.

6.8.3 Ensure that all the workmen engaged under him are selected through the screening system &
have undergone site SHE induction before assigning any task at the site.

6.8.4 Ensuring the workmen under him wear the necessary personal protective equipment (PPE)
respective to the job.

6.8.5 Eliminating all unsafe condition in their work area.

6.8.6 Keeping the work area neat and clean.

6.8.7 Ensure that underground utilities/cable must be surveyed to avoid damage to such utility cable.

6.8.8 Know the critical activities of his job based on the hazard/aspect identification and Risk/Impact
assessment and ensure implementation of the Risk/Impact control measures.

6.8.9 Participating along with the SHE Engineer/Officer or the committee members in the Project SHE
Inspection.

6.8.10 The site supervisor is to ensure that the correct permit to work documents are completed and
are authorized on the site where applicable. To follow all work permit requirements as per
Project’s SHE plan before starting of similar work.

6.8.11 To report all near miss cases/reportable LTI/dangerous Occurrences/fatality to SHE


representative immediately verbally & submitting the preliminary accident report within 12 hours.

6.8.12 Monitoring administration of First Aid.

6.8.13 Informing the concerned authorities as per the emergency response plan.

6.9 Safety & Health Officer (SHO)

6.9.1 Understand follow the applicable SHE requirements, laid down in project SHE plans,
Management system, manuals & Procedures, during execution of the work.
6.9.2 Carry out site SHE inspection and maintain the documents. And taking prompt action of site
inspection and hazard findings.
6.9.3 Provide guidance for safety precaution to be taken in construction work.
6.9.4 Ensure that all incidents (Near miss cases/Reportable LTI/Dangerous occurrence/
Fatality/Environment related incidents) are reported promptly.
6.9.5 Coordinate all investigation (Near miss cases/Reportable LTI/Dangerous
occurrence/Fatality/Environment related incidents) along with management team.
6.9.6 Conduct Emergency Mock drill on a regular interval.
6.10 SEnO/ EO (Sr. Environment Officer/Environment Officer)

6.10.1 SEnO& EO will be the key person in the project for environmental management. They shall
monitor closely the environmental aspects on the site environmental issue.

6.10.2 By co-ordination and communication with Site personnel and workers, ensure that the
Environmental requirements shall be followed at all levels.
6.10.3 To identify the need for and liaise with third parties for specialized training of personnel including
the training of sub – Contractors where required regarding environmental topics.
6.10.4 To prepare Environmental compliance report in coordination with Chief Environment Officer.
6.10.5 Preparation of different Job environmental analysis (JEA) for site related works in consultation
with site engineers under the guidance of Chief Environment Officer.
6.10.6 Routine monitoring to identify activities which are not in compliance with Environmental
Management Plan and propose corrective action for complying of the same.

6.10.7 Ensure the Implementation of corrective action.


6.10.8 Inspections and audits are carried out as per Contractual requirements.
6.10.9 SEnO& EO shall be responsible for liaising directly with Client representative for Environmental
issues/concerns.
6.11 LWO (Labour welfare Officer)

6.11.1 LWO shall be responsible for implementation of all legal requirement regarding Labour
protection and workplace policy as per employer/engineer requirement.
6.11.2 He will coordinate with government Labour officer/ Regional Labour commissioner regarding a
legal issue related to the project site.
6.11.3 He will be responsible for providing the Labour accommodation and other welfare facility for the
site execution team and workers.
6.11.4 He will be responsible for providing safe drinking water/ restroom for the site personnel &
workers.
6.11.5 He shall be actively participating to enforce the SHE requirement and participation in SHE
program.

7. QUALITY CONTROL AND ASSURANCE

7.1 Scope

Quality Assurance team will ensure that Work is being executed as per approved method
statement.

7.3.1 Quality Assurance Manager


(1) Shall have the overall responsibility and authority for activities of Project QA section. Establish
and ensure implementation of the quality management system in contract WDFC package STP5.
(2) Responsible for overseeing the overall implementation of the Quality plan of the contractor.
(3) Responsible for ensuring that the system is implemented in accordance with the standard.
(4) Authority to manage the system to bring non-conformance to the attention of the employer.
(5) Review the method statement/work procedure prepared by construction department for
inspection & test plan, checklists etc. Report on the performance of the Quality Management
System.
(6) Prepare the Management review report. Ensure that the QA team performs the Quality Audit/
Surveillance (Internally and externally). Approve the Vendor evaluation report (Subcontractor and
supplier).

7.3.2 QA/QC Engineer


(1) Identify material sources, and conduct the material test and ensure conformity with specification
& standards.
(2) Providing a checklist for major activities in order to ensure quality in the work.
(3) Conduct routine test on materials and workmanship as per ITP for conformance and maintain
records.
(4) Identify, Calibrate and maintain a master list of testing and measuring equipment, Statistical
analysis for product quality material (cube, plants, etc.).

8. WORK EXECUTION PROCESS

8.1 Job Site Planning & Preparation

8.1.1 Make sure there is space and firmed pathway for truck delivery, Hydra, truck erection
operation.

8.1.2 The plan for unloading and materials storage on the site should be suitable and dry
location. Materials shall be stored in designated areas for each building and clearly
identified for their location as per planning.

8.1.3 The surveying details of each location to be recorded incorporating reduced level and coordinates.

8.2 Receiving of Materials at Site

 All delivery materials to the site shall be informed to the site manager to have the plan
for unloading. Upon arrival of materials at the storage yard, the materials controller
will match delivery notes and shall verify the consignment.

 Unloading can be done manually by term with using 12-tonnes Hydra.A spreader
should be used for long steel components. Lifting nylon or cloth belts with suitable
SWL shall be used for unloading the

materials to minimize the damage. Be sure to hook belts to component with the right
number of points and position so that the load of the component itself do not damaged
or broken especially the coating/discolour.

 All materials receipt at the site shall be visually inspected by site supervisor/engineer
for any damage after unloading. Any damage shall be taken immediately, if possible, to
avoid any delay of erection

8.3 Steel Columns & Deck Sheet

8.3.1 Erection of columns:

 To determine that the foundation is square, measure diagonal dimensions to be sure they are of
equal length.

 Carefully check the location of all anchor bolts against the “Anchor BoIt” drawing. All dimensions
must be identical to assure proper start-up. Inspect all the nut & bolts have in pedestal is in free from
rust & concrete material.

 The erection area should be firm & compacted for safe vehicle operation including the periphery
area. Ensure that all erection activities carried under trained supervision. Ensure access will be
studied and prepared in advance for equipment arrival.

 Take careful note of any overhead electric lines or other utilities to avoid hazards and damage for
erection. The adequate capacitated equipment shall be in place for erection work.

 Provide appropriate PPE’s for workers. (Safety helmet, safety shoes, hand gloves, safety harness
etc.) Conduct a safety briefing for all workers along with site supervisor. Required area will be
barricaded properly. Consider the normal environmental condition for lifting activity.

 Ensure access is clear and adequate for material shifting from unloading location. Material
segregation will be done properly. Select the appropriate location for crane installation.

 Identify the correct column as per drawing and BOM and with the approval of make of the column.

 Check the position of crane, check the slings and packing to avoid cutting of slings at sharp edges of
column and check the equal loading on slings. Provision of access will in place i.e. rope ladder,
scaffold or man lift accordingly. Lift the column with gradual speed and fix to the nearest place of
pedestal.

 Arrange the bolts of pedestal as per water level measurement. Fix the column on pedestal and tight

procedure for further column erection.

8.3.2 Deck Sheet Fixing:

 The activity shall be carried out under close and continues supervision. Area where deck sheet fixing
activity shall be barricaded at ground to restrict the unauthorized entry.

 Proper roof access shall be provided to workers. Tag lines to be tied to the sheet bundle before
lifting. Fix the location to where to place the sheet bundle prior lifting.

 Employees handling deck sheet shall wear proper hand protection. Life line shall be provided at
working area for the workers while working at height. Life line to be tied with columns properly.

 Place the sheet at required location as per erection drawing and screw it. Place second sheet and
maintain proper overlap correctly. Continue the sequence. Ensure that deck sheet shall not be kept
in lose condition.

9. METHODOLOGY
The erection and installation of steel that according to the contract specification clause
9.1 Transportation of Fabricated Portal Frame
 The fabricated I-beam columns, rafter and truss by mean of the trailer.
The steel materials (fabricated portal frame) should be properly arranged
on the trailer by separately.
9.2 Unloading, Arrangement, Storage and Protect Material.
9.2.1 Unloading
 The fabricated portal frame unloaded by using 12-tonnes Hydra. Before unloading
materials out of trucks, the platform or access road must be prepared properly for
crane and trucks by term safely load.
9.2.2 Arrangement
 To avoid materials being moved so much on the job site that might
cause unexpected damages of paint or discolour and shapes, they
shall be unloaded and arranged close to the designed erection
point. The materials shall be stacked in location according to the
building/areas, should be near to the lifting position adjacent to the
area to be erected.
 Conditions of each job site, the materials arrangement plan may different
based on open clear area. The fabricated I-beam or columns should be
arranged close to their anchor bolt position. Rafter should be arranged to
ensure easy to assemble and move.
9.2.3 Storage and Protect Material
 To preventing and protecting materials from damages during storage that exposed
to environment factors such as storm water, dust, etc which can cause the rust,
stain, discolour, and etc, the proper storage should be provided to avoid the steel
materials damages, deformation and contamination. The tasks below should be
considered:-

 Dry area and stored above ground level with timber packing and
ventilated.
 Steel component or materials shall be stored separately for difference
sizes and types.
 The materials shall be kept free from the dirt, dust, grease and other
foreign materials.

9.3 Installation of Steel Column


Repeat procedure of erection columns and portal frames to complete frame.
9.3.1 Preparation
 After the columns are being distributed along the designated positions, the
materials need to be cleaned and assembled before erection. Level and
position of nuts and anchor, lifting weight and crane position shall be
checked.
9.3.2 Erection
 The 12-tonnes Hydra is set-up with the suitable position to lift the column
into position. Before major lifting, all the slings 1 rope is inspected by
riggers. The lifting 1 swing area will be barricaded to prevent unauthorized
works/personnel into the area. Slightly down column on cast anchor bolts.
The nuts will be tightened upon installation of column base plate into
anchor bolts in right position.

9.4 Erection of Portal/Beam Frame


The weight of portal frames is approximate 10 tones.

9.4.1 Preparation
 The scaffolding should be prepared for workers to perform rafter to rafter,
rafter to column connection including item 5.3.1. Temporary anchor points
should be arranged out of working area to avoid hanging materials can be
caught by temporary cable.

9.4.2 Erection

STEP 1

 The fabricated sections are joined together Beam at the site along the
gridlines.

STEP 2

 The connected frame will be slightly lifted by One Hydra (12-tonnes) into position
and bolts tightened. Two units of mobile scaffold platform to the joint height will be
pushed to place to do the nuts tightened.
 Workers on the ground will drive the fabricated steel (rafter) to the right position in
coordination with the crane. Crane is only released when all connection bolts and
nut adequate tighten.

9.5 Erection Sequence

9.5.1 Braced Bay Erection


 Brace bay will be erected in priority. After completion bracing bay frames, all
components such as brace rod, flange brace, etc of this shall be completed as
per shop drawing. Braced bay frames must be temporarily aligned before
installation purlins and bracing to avoid difficulties may get if conducting
alignment for remained frame.

9.5.2 Remaining Frames Erection

 Proceed with the erection of the remaining frames and additional bays. Eave
struts and peak purlins may be installed in the immediately bay between braced
bays to stabilize frames.

10 SAFETY HEALTH AND ENVIRONMENT (SHE)

10.3 General Safety Arrangements

10.3.1 Prior to any site construction work, all staff/operator/workers must undergo railway and
construction safety induction training.

10.3.2 All workmen must be screened, and ID card must be issued to them by the admin
departments.

10.3.3 All Engineers, Supervisors and deployed agency must be aware of SHE plan and other legal
requirements.

10.3.4 The site must be supervised by a competent supervisor who is familiar with all safety
precautions and work processes on the site.

10.3.5 This HIRA document, Method statement and structure design drawing must be communicated
to all responsible people on site by the responsible person directing the work and must be
available at the site.

10.3.6 Engineers, Supervisors, workmen must not be allowed to cross the railway tracks.

10.3.7 Barricading must be installed along for IR before the start of any work and additional safety
measures are required if working closer to 6m of the Indian railway.

10.3.8 Barricading must be installed around all construction site to prevent unauthorized access.

10.3.9 All Machinery deployed for WDFC Project must be inspected by P&M and SHE department
before first use. All vehicle must have a yellow sticker, green sticker displayed.

10.3.10 The structure should not be worked on during violent storms or high winds or any other
hazardous situation.

10.3.11 Exposed reinforcement heads shall be properly covered and the area is to be barricaded.

10.3.12 If night works are to be carried out, the area shall be provided with sufficient illumination of
lightning.

10.3.13 Site supervisor must ensure that prior to any structure work, that he is familiar with the
emergency procedures, has access to a vehicle/ambulance and has adequate first aid
facilities and trained first aiders. The first Aid equipment must be readily available at the site.

10.3.14 Working on gradients beyond any equipment’s capability shall not be allowed.

10.3.15 Drainage shall be provided to avoid stagnated water.

10.3.16 Every accident and near miss event shall be reported to the SHE Engineer & necessary
corrective and preventive action should be taken.

10.3.17 Smoking should be strictly prohibited at the site.

10.3.18 Findings from HIRA shall be monitored and implemented at site and Activity wise risk
assessment should be monitored on monthly basis.

10.4 Personal Protective Equipment (PPEs)

10.4.1 PPE must be worn by every person on the construction site and as a minimum must include
Safety Shoes, Safety Helmet, and Hi-Vi jacket.

10.4.2 In addition to the above, any other PPEs such as gloves, goggles, gumboots etc. required for
any specific jobs like rebar work, welding, and cutting, working at height, etc. shall also be
provided to all workmen and also ensure that all workmen use the PPEs properly while on the
job.

10.4.3 Site Supervisor is responsible for ensuring that PPE is available and is worn by all required.

10.5 Permit to Work System

10.5.1 The site supervisor is to ensure that the correct permit to work documents are completed and
are authorized on the site where applicable.

10.5.2 Deep Excavations.

10.5.3 Heavy lifting operations and lifting operations closer to the power line.

10.6 Excavation Safety

10.6.1 The site supervisor is to ensure that where the construction worksite has any hazards of falling
or sliding material or article from any bank or side of the excavation which is more than 1.5m
above the footing.

10.6.2 Excavated material is not stored at least 1 m from the edge of an open excavation or trench
and banks of such excavation or trench are stripped of loose rocks and other materials which
may slide, roll or fall into a construction worksite.

10.6.3 Where banks of an excavation are undercut, adequate shoring is provided to support the
material or article overhanging such bank.

10.6.4 In case any cable/utility is found while working inform the engineer immediately shall be
carried out in the presence of a representative from the concerned owning agency.

10.6.5 It shall be ensured that the moving dimension shall not be infringed.

10.7 Construction Site Warning Signs and Notices

10.7.1 The site supervisor is to ensure that suitable warning signs or notices shall be displayed or
erected at conspicuous places in Hindi, English or pictorial.

10.7.2 These signs or notices should give notice of hazards, rules and control measures to deploy on
site.

10.7.3 Danger warning signs should be positioned to warn the general public to avoid the
construction site.

10.8 Safe Working Practices Include

10.9 Housekeeping

10.9.1 The site supervisor is to ensure that all times the site is maintained to a good standard of
organization.

10.9.2 Ensuring the waste material is properly removed segregated and disposed of.

10.9.3 All material, tools, and equipment are kept in defined the locations, in a tidy and controlled
way.

10.9.4 Walkways are established and maintained that regular housekeeping is carried out by the
whole site team.

10.9.5 Lumber with protruding nails shall be either bent or removed and properly stacked. Un-packed
wooden strips shall be safely stored and disposed of.

10.9.6 Roads shall be kept clear and materials like pipes, steel, sand boulders, concrete, chips, and
brick etc. shall not be allowed on the roads which obstruct free movement of road traffic.

10.9.7 Empty cement bags and other packaging material shall be properly stacked and removed.

10.10 Hygienic Facilities

10.10.1 Drinking water, toilet and rest place shall be suitably located for the easy access to workers.
All the facilities shall be properly cleaned and maintained during the entire period of operation.

10.10.2 Sufficient toilet rooms with the proper facility to drain the sullage shall be provided at a
reasonably accessible distance.

10.10.3 All Drinking water locations must be legible marked “Drinking Water” in a language understood
by a majority of the workmen employed.

10.10.4 Surrounding areas of drinking water tanks/taps should be hygienically cleaned/maintained.

10.11 Environmental Aspects

10.11.1 Environmental aspects/impact shall be identified, and appropriate mitigation measures shall
be taken during execution of work as per Environmental and Social Management Plan (ESMP)
as part of Project Safety, Health & Environment Plan.

10.11.2 Rains: Rains create wet and slippery ground, loose soil and landslides, etc. depending on the
situation, the operator shall decide whether to continue working or not. When the soil is loose
and could cause danger to the crane’s stability, lifting work must be stopped.

10.11.3 Strong Winds: In strong winds, decrease the weight of the safe working load to improve
safety. If the wind intensifies, work must be stopped.

10.12 Waste Management

10.12.1 The S.A.F.E. Consortium shall comply in full with all relevant requirements of national
legislation pertaining to the management and disposal of waste.

10.12.2 The S.A.F.E. Consortium shall comply at all times with specific legislations and any other
relevant statutes.

10.12.3 S.A.F.E. Consortium shall make arrangements to deposit waste materials at construction sites
in safe condition.

10.12.4 Surplus inert material that cannot be used as infill or for other purposes on the project may be
deposited at unlicensed sites with the consent of the owner, if it can be retained indefinitely
without causing pollution or other environmental hazard or damage and if it does not create an
unsightly appearance.

The waste shall be sorted and segregated at the source of generation in the below mentioned

 Hypothermia - occurs when the body temperature drops from 98.6°F to 95°F or lower.
Typically, it is only a concern if you have been in cold water or outdoors without proper
protection for an extended period of time.
Symptoms: Shaking or shivering, Weakness or lethargy, Numbness or incoherence, Low pulse rate
 Frostbite - occurs when soft tissue, usually in the fingers or toes, freezes. While severe cases
can result in amputation, most cause no permanent damage if the skin is warmed up carefully.
Symptoms: Blisters, Numbness, Blackness, Tingling, throbbing, redness and/or swelling of affected
area upon re-warming.

OCP for hand tools


Hand tools used at project site shall be of good quality and appropriate for the work for which
they will be used. Hand tools include – axes, cleavers, hatchets, cold chisels, chisel cutters,
punches, crowbars and pinch bars, files, hacksaws, hammers and sledges, hand knives, lifting
jacks, hand hooks, pliers, wire cutters and nippers, screwdrivers, wrenches & spanners.

Hazards:

a) Tools falling down from heights

b) Hit by the sharp edges of the tools

c) Hit by flying objects

d) Slipping and falling due to the use of worn out tools

e) Broken of handle and others.

f) Improvised tools

Precautions:

1) Hand tools shall be issued through a responsible person. He shall inspect all the hand
tools before issuance to ensure that they are in good working condition.

2) All the supervisors and the safety representatives will look for defective tools being used
on the job on a continuous basis with specific reference to broken handles, blunt edges,
worn out heads, cracked parts etc. Such damaged tools found will be removed
immediately.

3) Hand tool should be tampered, dressed and repaired only by the trained person.

4) Improvised (Make shift or fabricated Unauthorized) tools will not be allowed and such
tools found at site will be removed and destroyed.

5) It is to be ensured that proper tools are used and in the right way. The misuse of
common hand tool is always a frequent cause of accidents. The examples for unsafe
practices are

 using pliers or wrenches as hammers,

 using screw drivers as chisels,

 using screw spanner or pipe wrench instead of double end or ring spanner,

 Screw driver applied to the object held in hands etc.

6) During toolbox talks the workers will be reminded about the perils of misusing hand
tools and they shall be prohibited to use defective tools.

7) Hand tools shall not be allowed to lie on floors, passageways, stairways, or in other
places where person have to work or pass.

8) The tools should not be left on scaffolds, overhead piping, or top of the step ladder and
in other locations from where they can fall on persons below.

 The excavation activity shall be done under supervision of site engineer/supervisor with maintain

 No excavated material will be allowed to get accumulated in safe condition away from the edge of

 Excavated material is not stored at least 1 m from the edge of an open excavation or trench and
banks of such excavation or trench are stripped of loose rocks.

 Safe access egress shall be provided for excavation pit or trench.

moving part of the machine or by the reversing of equipment/vehicle by providing signalmen.

 The Dump Truck that shifts the excavated material shall move only after lowering the hull /bucket
to original position; after completion of dumping the material if any.

OCP for Heavy Equipment


 The S.A.F.E. Consortium shall conduct regular inspections by P&M engineer to identify and
remediate any mechanical defects and wear and tear before the performance of the vehicles,
machinery or other equipment.

 In construction machineries, the project is expected to mobilized- dumpers and dump trucks, lift
trucks, mobile elevating work platforms, cranes, tipper lorries, 360° excavators, trenchers, trailers,
JCB, forklift etc.

 All lifting machinery e.g. Canes, Mobile crane(Hydra), fork lift, lift truck shall be certified by
competent (third party) person as per requirement of BOCWA, 1996.

 The copy of all valid certificates shall be kept at the main site office and copy of valid certificate
shall also be kept in the machinery.

 Periodicity of third-party inspection shall be done for the crane on every six-month duration copy
of the valid third party certificate shall be kept in the crane.

 Colour coding of the lifting appliances and gears shall be followed as per the colour code chart

 The competency of all operator of heavy machinery shall be checked thoroughly as per the format
of BOCWA, 1996.

 Driver/Operator shall carry his valid driving license with him during operation of vehicle &
equipments and copy of the same shall be kept in the vehicle /equipment along with all other legal
documents of the vehicle/equipments.

 Drivers shall also carry their original valid license for verification of the same.

 All operator shall undergo medical examination by the medical officer as per medical fitness
requirement of BOCWA, 1996.

 All construction machinery shall be inspected/checked at least twice in a month jointly by P&M
team as well as SHE team as per vehicle inspection checklist (WDFCC/P5/SHEP/10) attached in
Approved SHE Plan.

 Green/red tag shall be displayed on the machinery/lifting equipment for their safe use & not for
use.

 All Vehicles shall be fitted with audible reverse alarms and maintained in good working conditions.

 The trained flagman/banksman shall be provided in regular construction machineries as required


at project site for guiding the operator.

 Each construction machineries shall be deployed at Project site in sound working condition with all
legal documents as applicable in construction machineries.

 Schedule of preventive maintenance for each construction machineries shall be maintained by


P&M department.

 All records related to construction machineries shall be maintained by P&M dep’t and available to
the Engineer/Employer.

 All wheel mounted e.g. dumper, truck, tipper, trailers, JCB shall be checked for fitness certificate
from the Transport department and the copy of the same shall be kept in respective machineries.

 All drivers/operators shall be subjected to defensive training from authorized third party once in six
months.

 On regular basis all driver and operator shall be given induction training as well as refresher
training.

 Suitable signage/ poster shall be displayed near construction machinery during operation at
project site as applicable.

 Lifting zone shall be cordoned off during lifting operation with suitable barricading and flagman
shall be deployed.

 All machinery shall be fitted with the most up-to-date for the reduction of noise, and atmospheric &
other emissions.

 These should be maintained, serviced and repaired, where necessary, according to


manufacturers’ specifications and recommended schedules.

 Vehicle speeds on all construction sites to be limited to a maximum of 20 kmph at all times.

 Random alcohol monitoring of all operator/drivers shall be conducted by at site.

 Portable fire extinguisher shall be kept in driver/operator cabin and same shall be checked during
inspection schedule.

 The valid PUC certificate shall be checked before mobilization of wheel mounted construction
machineries.

 All power tools of 220v will be properly grounded and insulated with necessary ELCB protection.
 All tools shall be registered and identified and subject to planned maintenance, and their record
shall be maintained by the Store In-charge.
 An authorized person (Electrical engineer) at site will check all portable power tools before
releasing it to the site use.
 It will be the responsibility of the site Safety Representative to ensure that all power tools are
regularly inspected along with electrical team and a record shall be maintained.
 If a power tool is found to be unsafe to use during the regular inspection, it shall be marked/
tagged to that effect and returned to stores for repair or replacement.
 Power cables provided to the tools shall not have any joint and the cables will be laid in such a
way that it does not present any trip hazard.
 All power cables will be provided with industrial plug and sockets as applicable.

 The guards provided in the power tools shall not be removed either by the technicians or by the
maintenance electrician during its maintenance or inspection only.
 Always fit or disconnect accessories only after switching off the power and do not forget to put the
guards of the equipment in position before using it.
 The power tools shall not be dragged using its cord / flexible cable and be stored properly to
prevent any damage.
 Cartridge operated power tools, if any, will be used only by trained technicians.
 Supervisor will ensure that correct wheels for the type of machine, speed and material to be cut
are ordered and issued for the work.
 The supervisor shall ensure that only physically fit and trained operatives are deployed to work
with abrasive wheels and discs and required PPE as appropriate for the work and machine are
issued and used by workers and operatives are not wearing loose clothing.
 The operative will visually check equipment and discs / wheels for visible sign of damage before
use, ensuring operative speed indicated is correct for the machine.
 User shall not use undue pressure on the equipment.
OCP for Work at Height:
 Medical fitness of worker shall be checked by the medical officer as per criteria given in BOCW
rule, 1998 before deploying them for height works.
 They will provide the special height work training including the safe use of required PPE and
encourage them to regular use of PPE during working at height.
 Site Engineer/supervisor of the respective location shall check the non-slippery footwear of the
workers and same shall be monitored by Site SHE personnel on periodical basis.
 Employees deployed for height works shall be selected persons who are specially trained for
working at heights.
 A Permit to Work for working at height shall be issued before commencing any work at height
involving the use of fall protection equipment e.g. full body harness, fall arrestor, safety net (if
required) etc.
 Workers shall carry tools in a bag during working at height. Throwing up or throwing down tools is
strictly prohibited.
 On horizontal movements the lanyard is hooked to the static lifeline rope. Static lifeline is of 8mm
wire rope or a certified and tested Horizontal Anchorage Life Line made from 25 mm polyester
webbing material.
 An employee (look out) should be posted at ground level to clear and caution and hence prevent
persons from coming / passing under the height work area.
 Suitable caution boards will be displayed to warn unauthorized persons from coming near below
the height work area where they may get injured due to any accidental fall of material from height.

Safe Usage Of Ladders

 It is prohibited to use ladder as a working platform for height work.


 Ladder will only be used for access & egress for working platform for height work.
 When a ladder is used it must be adequately secured (anchored) at the top & at the foot to avoid
slipping.
 The user must ensure that he maintains 3-point of contact with ladder and throughout the duration
the ladder shall be used.
 Metal ladders should not be used in the proximity of live electric wires.
 Ladder shall be given an inclination not steeper than 1 in 4 (1 horizontal 4 vertical).
 The ladder should extend at least 1m above the work place / platform/ Stepping off point.
 Ladders shall be maintained free of oil, grease and other slipping hazards.
 Wooden ladder shall not be allowed at the working site.

Safe Usage of Scaffolds

 All scaffold materials shall be used as per the IS standard.


 Scaffold structures shall be provided on a solid base avoiding pavements & manhole covers etc.
 All scaffolds and work platforms erected or altered, shall be inspected on periodical basis and
dismantled under competent persons (Scaffold Supervisor) who have been formally authorized by
S.A.F.E. Consortium.
 The scaffold shall be inspected with maintain the checklist point & inspection report shall be
recorded for all scaffold.
 Scaffolding Tags: Green Tag declaring Safe for use & Red Tag declaring unsafe for use or
scaffolding erection / dismantling in progress.
 The site engineer and Site SHE personnel shall check the scaffold before displayed suitable tag
on scaffold.
 All Scaffolds, work platforms used to prevent fall from height shall use internal ladders for access.
 For the working platform provided over the scaffold provision of toe board / hand rail and mid rails
shall be ensured.
 Usage of bamboo & wooden scaffolding is prohibited. Only IS standard steel pipe for scaffolding
shall be permitted.
Safe Usage of Working Platforms

 All works to be carried out over water or at a height greater than 2.0m above the ground level
must be performed from a properly built up and maintained working platform.

 The working platform shall be made with maintain the working platform width, hand rail, toe guard
etc. according to standard.
 The mid rail that is provided between the platform floor and the top rail shall be on the inside of
the posts.
 Mobile scaffolding shall be fitted with lockable wheels of a minimum 130mm in diameter.
 No person will ride the mobile scaffold while it is being shifted or moved.
 All working platforms shall be clean and free from grease, oil, rubble, debris or rubbish to ensure
safe movement of personnel performing the works.
 Good practices poster shall be displayed at the work location for communication and awareness
of the workers.
OCP FOR CRANE OPERATION
 Operator who are above 21 years of age and possess a valid Heavy Transport Vehicle (HTV)
Driving license shall only be permitted to operate the crane.
 The operator must possess the knowledge of the inherent risks involved in the operation of lifting
appliances.
 Trained crane operators shall only be deployed for the job.
 Operator shall always carry his valid license and the fitness certificate of the crane he operates
 The laminated photocopies of fitness certificate issued by competent person, the Employers’
approval letter, the operators’ photo, manufacturer’s load chart and Competency certificate shall
always be pasted on the visible surface of the lifting appliances.
 Assembling, maintenance and disassembling on the equipment shall be done as per the
manufacturer’s manual.
 Overloading shall never be allowed at site.
 Ensure the ground area is good enough for the crane before marching or parking it.
 Crane shall be stationed on a firm level ground otherwise in case of crane operation on out of
level ground causes side loads tend to develop in boom thereby causing the crane to lose its
capacity leading to crane toppling accidents.
 Proper boom angle and radius shall be followed to lift the load. Refer the load chart given in the
crane manual or fixed inside the crane cabin.
 Cranes shall not be parked near the edges of pit or excavations.
 Limit switch of cranes shall be tested frequently for ensuring its proper functioning.
 All hydraulic piping and fittings shall be maintained leak proof.
 No load shall be slewed over public areas without stopping the pedestrians and road traffic first.
 The swinging area of the crane shall be cordoned. The Crane helper shall ensure no trespassers
enter in this vicinity.
 The operator shall have a clear view of the operation throughout the hoisting period.
 A trained signalman shall give the signal for crane operation.

OCP for Welding work:


 The frame or housing of the welding machine shall be properly grounded.
 The welding machine shall be protected by a properly sized fuse and circuit breaker on an
independent circuit.
 Transformer used for electrical arc welding shall be fixed with Ammeter and Voltmeter and also
fixed with separate main power switch.
 Welding shall be done in an area with adequate ventilation.
 Connection of the welding cable with the output pole of the welding machine shall be suitably
done with proper fittings. Welding cable joints shall be made using ferrule and the joint shall be
insulated.
 Welding grounds and returns should be securely attached to the work by cable lugs, by clamps in
the case of stranded conductors, or by bolts for strip conductors. The ground cable will not be
attached to equipment or existing installations or apparatus.
 Electrode holder shall be of good quality with proper insulation.
 The welder should never be allowed to weld when he is wet. Electrodes must not be changed with
bare hands. Avoid being in contact with water or wet floors when welding. Use duckboards or
rubber protection.
 Usage of proper personal protective equipment by the welder (welding hand gloves, insulated sole
shoes, welding apron, safety helmets with welding shield) shall also be ensured.
 Adequate ventilation shall be ensured at the area where welding operation is being performed to
prevent welding fumes from accumulating.
 When not in use, the current to the holder and electrode must be turned off
 Combustible and flammable materials shall be removed from the place before starting the welding
operation.
 To prevent musculoskeletal injuries welders must not work in one position for long time periods

Site SHE personnel.

 Smoking or any other hot work is strictly prohibited while painting or area where paint container(s)
are kept or stored.
 Painters should wash their hands and faces before eating, drinking, or smoking.
 Painters should change into clean clothing before leaving the worksite.
 Paint shall be store at designated place with taking care of safety measures as required.

10.18 Safety bulletin

SAFETY BULLETIN – LOADING AND UNLOADING OF MATERIAL

 Loading and unloading shall not undertake at carriageways obstructing the free flow of vehicular traffic and encroachment of existing roads.
 Loading and unloading heavy equipment is to be done in compliance with manufacturers guidelines.
 Park the truck and trailer on level ground to ensure that equipment can be loaded or unloaded without tipping or sliding.
 Apply parking brake. This will ensure that the truck and trailer do not move while loading & unloading.
 Ensure the loading ramp or blocks are safe to use. They must be strong enough to withstand the weight of the equipment/material being transported to ensure
they will not collapse and potentially crush the worker or cause damage to equipment/material.
 When loading, ensure that the machine is lined up precisely with the deck. This will provide easy and safe unloading.
 When the machine/material is in proper position on the deck, apply parking brakes and ensure that properly rated chains/lashing belt and boomers are used to
secure machine to deck and will not move during transportation.
 All attachments should be lowered on and resting on the trailer deck and any loose materials or parts should also be chained or strapped down as per load
securement regulations, minimizing the risk of personal injury or property damage.
Manual Loading/ Unloading

 Assess the load that includes its weight, shape and size, suitability of griping, outer surface etc. with respect to his own limitations.
 Access and egress is to be maintained clear.
 If you are required to carry something that is heavy or awkward, always assess other alternatives, for example using a mechanical aid /trolley. If this is not
possible, ask someone else to help you.
 Proper manual material handling technique to be adopted.
 Proper material stacking to be done under safe height.
 Safe arrangement to be done for lifting the materials at height, if any.
 Required PPE to be used.
Loading /unloading by the use of Crane/Hydra

 All lifting activity shall be carried out by trained and experienced workers under competent supervision.
 Lift study to be done before lifting activity
 Lifting permit to be obtained before lifting operation if heavy lifting to be carried out.

 Lifting area to be cordon off, the crane helper to be ensured no trespassers enter in this vicinity.
 Lifting materials weight checked before lifting and certified/ good condition/ rated lifting tools and tackles used with colour coding.
 Only competent operators are authorized to operate the crane/hydra
 Daily checklist (pre-operation) to be maintained by operator.
 Operators to make sure that Crane/Hydra are positioned properly and angle of boom is proper for weight being lifted
 Outriggers are provided with crane, shall be fully extended.
 No person shall be positioned under any suspended load or between any suspended load and fixed objects.
 Tag line to be used to avoid the swing of load
 No load shall be slewed over public areas without stopping the pedestrians and road traffic first.
 A trained signalman shall give the signal for crane operation.
 All lifting tools provided with a safety latch and no equipment used under high wind/ heavy rain or if the weather condition is bad.
 First aid box and portable fire extinguisher to be kept inside the cabin.
