COMPUTERIZED MCQ SERVICES CONTRACT

1. SERVICES PROVIDED:

Provider agrees to provide access to computerized Multiple-Choice Questions (MCQ) for health
professionals. The services include:

MCQs you would like to access for each category:

Allied Health MCQs: 500 AED

General Practitioner (GP) MCQs: 800 AED

GP Dentist MCQs: 800 AED

Specialist MCQs: 1000 AED

Consultant MCQs: 1300 AED

1.3. Access Validity: Three Months

Government fees paid by customer.

2. PRICING:

Customer agrees to pay the following fees for the computerized MCQ services (including 5% VAT):

- Access to Health Professional MCQs for: ______________.

- Total Amount: Total Price including VAT: __________________AED.

3. PAYMENT TERMS:

Customer agrees to pay the total amount specified above (including VAT) before access to the MCQs
is granted. Payment shall be made via bank transfer, payment link, cash, tabby ,cashew,tamara.

4. ACCESS TERMS:

Upon successful payment, Provider will provide the Customer with a username and password within
two working days. The provided access will be valid for a duration of three months from the date of
activation.

5. CANCELLATION POLICY:

Any cancellations after payment is made will not be eligible for a refund.

6. GOVERNING LAW:

This Contract shall be governed by and construed in accordance with the laws of Abu Dhabi Court.
