Should your doctor’s examination not indicate any issues, with great probability you currently do not have an
increased health risk, and you have the appropriate health and fitness level to:

If your health status declines, you should consult your doctor to see if you can participate in sports. Should you
have a cold or fever, you should take a break from training.

Please repeat the questionnaire once a year or when you have taken a break from sports for longer than 6
months. In addition, for your own safety we encourage you to have physicals on a regular basis (at least every 2
years) to verify that you are fit enough to participate in sports. Information on what should be covered at such an
