It is for the information of all the registered candidates desirous of seeking admission to
Nursing, Physiotherapy and Paramedical Courses in all Government / Private Institutions in the State
of Haryana for the Academic Session 2023-24,2023 4, that all the candidates, at the time of document
verification must bring the following original certificates/document/testimonials along with two sets
of duly attested photocopies:
1. Matriculation Certificate/High School Certificate/CSE/CBSE Class 10th or its equivalent
certificate as proof of date of birth.
2. Pass certificate/DMC of qualifying examination i.e. 10+2.
3. Three copies of the recent photographs (passport size) self attested used for CET form.
4. GNM/B.Sc. Nursing/Post Basic B.Sc. Nursing including DM DMC,C, Registration Certificates,
