DEED OF ASSIGNMENT

This Deed of Assignment (“Deed”) is entered into by and between:

as the “ASSIGNOR”,

Mrs. Presentacion S. Ramos, and hereinafter referred to as the “ASSIGNEE”

WITNESSETH

That the ASSIGNOR is a creditor of ________________________, a corporation duly organized

and existing under Philippine laws and with business address at the________________;

That said corporation has an outstanding obligation to the ASSIGNOR in the amount
of__________________;

That the ASSIGNOR, in turn, has an outstanding obligation to the ASSIGNEE, and in partial/full
payment and satisfaction thereof, the ASSIGNOR does hereby ASSIGN, TRANSFER, and
CONVEY unto the ASSIGNEE all its rights and interests in any and all amounts owing to it from

That the ASSIGNEE does hereby accept this assignment as partial/full payment of the
ASSIGNOR’S outstanding obligation to the ASSIGNEE.
