AFFIDAVIT FOR GAP PERIOD

I, JAMMIKUNTLA SRINIVAS S/o, LATE JAMMIKUNTLA THIRUPALAIAH, Occ. Freelancer and resident
of H.No-1-7-78/3, F1B, Venkata Srinivasa Residency, Kamalanagar, kapra, Ecil Post, Hyderabad-500062,
do hereby solemnly state & affirm as under:-

(1) That I am a permanent resident of above said address since 35 years.

(2) That I have Cleared TSLAWCET 2022 and seeking admissions in to a reputed college.
(3) That I have passed Bachelor’s of Technology in the year 2011 from TKRCET Hyderabad affiliated to
JNTUH University.
(4) That I have not joined/admitted in any School/College/Institution due to my father’s death in Dec
2010 followed by family responsibilities.
(5) That there is a GAP in my studies from 2011 to 2022.
(6) That during this period I was not involved in any offence or in an illegal activity and that any
Criminal case is pending against me in any court of law.
(7) That I command a good reputation and respect in general public.
(8) That I have not availed post matric scholarship for the same program name or course from any
College/ University/ Institute.

Verified that the contents of my above said affidavit are true and correct to the best of my knowledge and
belief and nothing has been concealed or misrepresented therein. In case the above facts are found in
correct at any stage then my admission can be cancelled by the University.
