DEFINITIONS
Major Classification. – Organizations registering under this program shall be classified primarily into:
(a) YOUTH ORGANIZATION – shall refer to an organization with members in good standing entirely composed of
the youth, whose ages range from fifteen (15) to thirty (30) years old with a core advocacy that serves the
youth.
(b) YOUTH SERVING ORGANIZATION – shall refer to registered organization with members in good standing 1
whose principal programs, projects and activities are youth-related and youth-oriented, and whose
composition are not limited to the youth. The organization should be duly recognized or accredited either
by proper national or local government registering entities.
Organizational Level. – Registering organizations shall be classified according to their scope of membership and
operations, as follows:
(a) NATIONAL ORGANIZATION – a youth organization or youth-serving organization with members coming from
the three (3) island groups, and whose major programs are national in nature and scope
(b) INTER-REGIONAL ORGANIZATION – a youth organization or youth-serving organization with members
coming from at least two (2) regions but not from the three (3) island groups, and whose major programs
are either regional or national in nature and scope.
(c) REGIONAL ORGANIZATION – a youth organization or youth-serving organization with members coming from
at least two (2) provinces within a region and whose major programs are regional in nature and scope
(d) PROVINCIAL ORGANIZATION – a youth organization or youth-serving organization with members coming
from at least two (2) cities and/or municipalities of a province and whose major programs are provincial in
nature and scope
(e) CITY/MUNICIPAL ORGANIZATION – a youth organization or youth-serving organization with members
coming from at least two (2) barangays of a city or a municipality and who’s major
Sub-Classification. – Registering organizations shall be further classified according to the majority of its membership,
as follows:

(a) COMMUNITY-BASED ORGANIZATION – refers to a youth organization or youth-serving organization whose
programs, projects and membership are based only in a certain locality or community.
(b) FAITH-BASED ORGANIZATION – refers to a youth organization or youth-serving organization whose
membership belongs to a certain religion or sect or religious belief.
(c) SCHOOL-BASED ORGANIZATION – refers to a youth organization or youth-serving organization whose
membership are students belonging to schools, universities or colleges legally recognized by the state.
(d) CONSORTIUM/ FEDERATION – a youth organization or youth-serving organization whose members are at
least two (2) youth organizations and youth-serving organizations which agreed to consolidate their efforts
to pursue a common interest.
BRIEF DESCRIPTION & OBJECTIVES (maximum of 300 words only)
The Kamunggay-Lusbak Youth Association (KLYA) is a vibrant and progressive organization devoted to the
empowerment and development of the youth in Sitio Kamunggay and Sitio Lusbak. The association is built on the
pillars of leadership, community service, cultural enrichment, and environmental sustainability, aiming to shape the
future leaders of our community. KLYA strives to provide a nurturing platform for young individuals to develop their
skills, realize their potential, and contribute positively to society. Their mission is to inspire and equip the youth with
the necessary tools to become proactive, informed, and responsible citizens. KLYA envision a community where the
youth are empowered to lead, innovate, and drive sustainable change, ensuring a prosperous and harmonious
future for the youth.

SUBMISSION OF REQUIREMENTS
The documentary requirements of registering youth organizations and youth-serving organizations may be
submitted to the appropriate office:
(a) Those municipal or city organizations shall apply for registration at their respective Municipal or City Youth
Development Office, the designated personnel, in the absence of an LYDO, or its equivalent;
(b) Those provincial organizations shall apply for registration at their respective Provincial Youth Development
Office, the designated personnel, in the absence of an LYDO, or its equivalent;
(c) Those regional and national organizations, and consortiums and federations shall apply for registration to
the National Youth Commission.
In the absence of a Local Youth Development Office or its equivalent in a certain local government unit, the
organization may apply for special registration and verification with the National Youth Commission. The president
of the organization shall put into writing, addressing to the NYC Commission Proper through the YORP Secretariat,
their request for special registration and verification:
• NYC Area Offices in Baguio, Naga, Cebu, Zamboanga, Davao and Cagayan de Oro
(Addresses of area offices are up in our website at www.nyc.gov.ph)
(1) The information contained herein and the attachments are true and correct;
(2) We consent that the information supplied in this Application for Registration be posted online or printed as
part of the official list of registered and verified youth organizations and youth serving organizations to be
published by the National Youth Commission;
(3) We have read and understood in full the Policy Guidelines in the Registration of Youth Organizations and
Youth-Serving Organizations (2017 Youth Organization Registration Program Guidelines), and agree to
abide by its provisions;
(4) Should the organization commit any violation of the Philippine Constitution or laws of the land or rules and
regulations of duly constituted authorities and/or violation to any policy, resolution, or rules and regulations
of the National Youth Commission, our registration may be suspended or revoked.
We, the undersigned, expressly authorize the National Youth Commission or its representatives to use, share and
process personal information that we have provided, shared or declared in this form/document for any lawful
purpose.

Article I. Name
Section 1. This organization shall be known as Kamunggay-Lusbak Youth Association
a. Throughout this document, this organization shall be referred to as KLYA
The secretary shall keep the organization's membership roster. All official
decisions, processes, and actions of the organization, including but not limited
to meetings, elections, and communication, must be accurately and up-to-
date recorded by the Secretary. In addition, the secretary will be responsible
for maintaining the organization's bylaws and constitution, making sure they
are regularly examined for correctness, applicability, and conformity with legal
requirements. They will assist in implementing any suggested changes or
additions to the bylaws and constitution in accordance with the established
protocols.
Section 7. There shall be an advisor of the organization who shall be selected by the
Board of Officers and is selected by qualifications on a term of service.
a. Qualifications of Advisors:
i. The advisor shall be a resident of either Sitio Kamunggay or Sitio Lusbak
(Purok 3 and Purok 4 ) of Barangay Bacong in the municipality of Anda
in the province of Bohol.
ii. The advisor shall be a registered voter in the barangay.
iii. Advisors shall be individuals who have demonstrated a commitment to
the development and well-being of youth within the community.
iv. Advisors should possess expertise, experience, or a strong interest in
areas relevant to the mission and objectives of the organization, such as
education, leadership development, community engagement, or youth
advocacy.
b. Selection of Advisors:
i. Advisors may be nominated by members of the organization or
identified through outreach efforts within the community.
ii. The selection of advisors shall be conducted through a transparent and
inclusive process, which may include applications, interviews, or other
appropriate methods determined by the organization's leadership.
iii. The final selection of advisors shall be approved by 50% of the members
of the Board of Officers
c. Term of Service:
i. The term of service for advisors shall be three years and is renewable
upon mutual agreement between the advisor and the organization.
ii. Advisors may be removed from their position by the Board of Officers
and a written petition by 50% of the members of the organization for
reasons such as misconduct, neglect of duties, or inability to fulfill
responsibilities.
d. Compensation:
i. Advisors shall serve on a voluntary basis and shall not receive financial
compensation for their services.
ii. However, advisors may be reimbursed for reasonable expenses incurred
in the performance of their duties, subject to the approval of the
organization's leadership and in accordance with the organization's
budget and financial policies.
Section 2. Any member who are able to meet the following qualifications shall be
eligible to be elected to any position within the organization:
a. Age Requirement:
i. Must be within the eligible age range specified in the organization's
by-laws, typically within the youth age bracket of 15-30 years old.
b. Membership in the Organization:
i. Must be a registered member of the Katipunan ng Kabataan and in
good standing with the Youth Organization Registration Program.
c. Commitment to the Organization's Mission and Values:
i. Demonstrates a strong commitment to the goals, objectives, and
values of the organization, as outlined in its constitution and by-laws.
ii. Shows a genuine interest in serving the community and advocating
for the welfare of youth.
Section 3. The term of office of elected officers shall be three (3) years. Elections shall
be conducted every three (3) years.
Article V. Meetings
Section 1. The regular meeting of the Board of Officers shall be held quarterly. In
evidence of a lack of a quorum the regular meeting will be postponed until further
notice of the secretary.
Section 2. The members of the Board of Officers must be notified about the upcoming
meeting no less than two (2) days before the meeting including the place, time and
the agenda of the meeting, if it was not decided on the previous date. Notification
of the special Meeting that is called to solve urgent issues can be sent by electronic
messaging a day before the date of the Meeting. It is possible to make decisions on
urgent issues by means of surveying the members of the Board of Officers through
electronic messages. Decision which is made in such a manner must be recorded and
affirmed on the next meeting of the Board of Officers
Section 3. The Board of Officers makes its decision by open simple majority voting of
the present members. If all attempts to break the tie through discussion,
reconsideration, and additional voting rounds fail, the Chairperson, as the presiding
officer, may cast a tiebreaker vote to decide the matter. It’s essential that the
Chairperson's tiebreaker vote is exercised impartially and in the best interest of the
organization, without personal bias or preference.
b. Committee on Education
i. Design and implement educational initiatives and workshops to
enhance the academic and intellectual growth of youth members
ii. Committee on Socio Cultural Activities
iii. Committee on Environment and Disaster Risk Reduction and
Management.
Section 2. Every committee shall be composed of a Chairperson and at least two (2)
members. The following are the qualifications to become a committee chairperson
and a committee member.
Section 2. During the investigation, the officer who is under such will be suspended
until all due processes are undertaken by the Board of Officers.
Section 3. If proven unworthy of his post, with due process and evidence, the officer
involved will be impeached upon the approval of the Board of Officers. Their decision
shall be final and irrevocable.
