Re: Ms. Shauna Mendes, DOB: 13/01/1997

I am writing to refer Ms. Shauna Mendes, a 25-year-old female, diagnosed with bacterial
keratitis to your care for further ophthalmological assessments.

On 19/03/2022 Ms. Mendes presented with a tearing feeling and a foreign object
sensation on her right eye for the past 3 days accompanied by redness, pain and difficulty
opening the eyelid for 2 days. On physical examination, inflammation, scarring and
rashes were visualized in the cornea. Therefore, A tear sample collection was taken. Ms.
Mendes was prescribed Tobramycin 14 mg/ml eye drop every hour and Cefazolin 50
mg/ml eye drop every hour.

Ms. Mendes Medical history is relevant for Myopia, seborrheic blepharitis and allergy to
dexpanthenol drop eyes. Her current medications are TheraTears lubricant eye drops,
sulfacetamide 10% topical use and Pantropazole as needed. Ms. Mendes, reported to have
worn old contacts lens 3 days back.

On today’s visit, the pathology reported: Pseudomonas bacterial infestation. Ms. Mendes
was instructed to avoid aye manipulation as well as to continue with the same medication
plus prednisolone acetate 1% every 6 hours.

I would be grateful if you could provide Ms. Mendes of further ophthalmologic


assessment management for her diagnosis.
