Environment

Voicemail system type A4635 all releases

Scenario

Sometimes without any reason the voicemail system reset with following incident:
000/28/-/---|=3:3001=IO transm. fault 0 3 0 4 20a 1285

Resolution

Resolution Analysis

Retrieve the following informations:

1. status logs from A4635 via SMT menu 13-2 (review status logs)
2. log with root account via telnet on the 4635 , run the following command and capture the result
3. in the status logs we can only see that the system was restarted.
4. in the /var/log/messages, we can see the following informations which is the root cause of the issue.

via SMT menu 17-4 (Internal Access Server Administration), disable "Start Internal Access Server"
