1. GENERAL
1.1 General
This requisition covers the minimum requirements for the erection, installation,
construction, site inspection, site testing and pre-commissioning & commissioning
test of MEPI Building Works (hereinafter called the work) to be performed and
completed by SUBCONTRACTOR for mining facilities located at Morowali, Central
Sulawesi. SUBCONTRACTOR shall perform and complete the Work in strict
accordance with the requirements contained in this Requisition.
1.2 Definitions
PURCHASER – means PT PP (Persero) Tbk and its designee who acts on its
behalf for Service Order.
OWNER – means PT VALE INDONESIA, the Owner for whom the Purchaser
performs work in which Services are to be incorporated.
SUBCONTRACTOR – means the firm or company whom Service Order is to be
placed.
1.3 Language

English shall be used in the documents, report and drawings to be prepared for this
project.

2. SCOPE OF WORK AND SUPPLY


SUBCONTRACTOR shall be responsible for all work items required to perform and
complete the erection, installation, construction, site inspection, site testing and pre-
commissioning & commissioning test assist work in accordance with the
requirements of this Requisition and the related documents shall refer to APPENDIX
1: List Scope of Work and Supply and APPENDIX-3: Bill of Quantity (BOQ).
SUBCONTRACTOR shall provide labor, facilities, tools, consumables, testing and
inspections and all other items and services necessary to complete the Work at his
own cost and responsibility.
The following items shall be included in SUBCONTRACTOR’s scope:
2.1Direct Work & Indirect Work
In general the scope of work is defined as follows:
 Electrical Power Panel
 Lighting & Recepetacle
 Fire Detection & Alarm System

Detail of Scope of Direct & Indirect Work shall refer to APPENDIX-1: Scope of
Work and Supply and APPENDIX-3 : Bill of Quantity (BOQ). SUBCONTRACTOR
shall provide the Work at his own cost and expenses.
2.2 Material
Material loading and unloading (at site), handling, storage and protection shall be
SUBCONTRACTOR’s responsibility and shall be performed in the qualified manner.
2.3 Pre-commissioning and Commissioning
SUBCONTRACTOR shall provide his manpower on Pre-commissioning and
Commissioning stage to assist the experts. The number, qualification and
assistance duration of this stage shall be as PURCHASER’s request. The
manpower shall be completed with needed tools, apparatus, and other items
necessary to perform the Work.

3. MANPOWER
3.1 SUBCONTRACTOR shall provide all of direct manpower including unskilled and
skilled labor that are needed to perform and complete the work.
3.2 The manpower mobilized by SUBCONTRACTOR shall have sufficient skill &
certified in accordance with his specialization or job description. If any manpower is
deemed unacceptable or has no skill in accordance with his job description, in the
event the personeel of SUBCONTRACTOR is or are unable to pass the medical
check up, SUBCONTRACTOR shall replace the person with acceptable and
capable manpower immediately.
3.3 SUBCONTRACTOR shall gurantee that the manpower has good medical check up
history also capable to perform the work and shall provide to the PURCHASER the
list of Manpower within 1 months before the manpower loading schedule.
3.4 SUBCONTRACTOR shall provide its healthy personeel for each position of
Manpower in order to ensure that manpower will pass the medical check up
requirement and procedure from the Owner.
3.5 SUBCONTRACTOR shall provide medical check up for manpower until the
manpower has decided to be FIT to work by the Owner. if the result of manpower
UNFIT to work SUBCONTRACTOR shall be replace their manpower with
acceptable manpower immediately as per above period.
3.6 SUBCONTRACTOR shall provide all of safety accessories, necessities, insurance
and others for his manpower.
3.7 SUBCONTRACTOR shall complete his manpower with necessary certificate related

to his job in accordance with the law or regulation.


3.8 SUBCONTRACTOR shall ensure all non-skill worker will be recruited from local area
nearby project site.
3.9 If it necessary and/or as PURCHASER’s request, SUBCONTRACTOR shall
mobilize additional manpower to achieve work progress at his own cost and
expenses. If SUBCONTRACTOR cannot mobilize additional manpower,
PURCHASER has the right to mobilize manpower and the cost related to manpower
mobilization will be charged back to SUBCONTRACTOR.
3.10 SUBCONTRACTOR shall follow and comply to mobilisation process as per
APPENDIX – 6 Construction Master Schedule.
3.11 SUBCONTRACTOR is acknowlegde that unability to provide the capable, health,
skill of Manpower may be subject to LD Delay implications.

4. CONFLICT
In the event of conflict amongst contract documents, SUBCONTRACTOR shall notify
the PURCHASER and obtain written clarification of the points in question.
SUBCONTRACTOR shall not commence Work before receiving approval from
PURCHASER.

5. TESTING AND INSPECTION


5.1 SUBCONTRACTOR shall be responsible for quality assurance and quality control
program during execution of the Work to maintain good quality and workmanship.
5.2 Details of testing and inspection procedures shall be prepared by
SUBCONTRACTOR and submitted to PURCHASER for approval prior to start the
Work.
5.3 SUBCONTRACTOR shall prepare inspection and or test report for review.
5.4 SUBCONTRACTOR shall perform field-testing and inspection in accordance with
the specifications and drawings of the items directed by PURCHASER.
SUBCONTRACTOR shall be responsible to provide all apparatus, tools and
materials required for testing and inspection.
5.5 Cost and expenses related to testing, inspection, recording and reporting including
apparatus, tools, and materials shall be on SUBCONTRACTOR’s responsibility.

6.1 SUBCONTRACTOR shall perform and complete the work in accordance with
APPENDIX- 6“Construction Master Schedule”
6.2 Based on the Construction Master Schedule and construction milestone required,
SUBCONTRACTOR shall prepare and submit a Construction Detail Schedule.
6.3 If SUBCONTRACTOR’s work performance does not meet the schedule,
SUBCONTRACTOR shall make necessary efforts to recover the progress schedule
at SUBCONTRACTOR’s cost.

7. SAFETY AND SECURITY PROGRAM


7.1 SUBCONTRACTOR shall propose Safety Program that will be applied in the
performance of the Work for PURCHASER’s review and approval in accordance
with PURCHASER’s SHE Project Plan. All of SUBCONTRACTOR’s staff and labor
shall aware of the Safety Program. SUBCONTRACTOR shall provide all the
equipment, tools and apparatus required to complete the Safety Program.
7.2 SUBCONTRACTOR shall be responsible for his property security (material,
construction equipment, tools, etc.). Any loss of SUBCONTRACTOR’s properties
shall be SUBCONTRACTOR’s responsibility.
7.3 All cost and expenses related to the safety and security program shall be
SUBCONTRACTOR’s responsibility.
7.4 SUBCONTRACTOR shall provide the medical check-up result for each manpower
(direct & in-direct) from the trusted laboratory that has been recommended by
owner.

8. CONSTRUCTION EQUIPMENT & TOOLS


8.1 SUBCONTRACTOR shall provide construction equipment & tools that are needed for
execution and completion of the Work, including its accessories, consumables,
spare parts, maintenance, permission, and others related to the operation of the
construction equipment at SUBCONTRACTOR’s own cost and expenses.
8.2 All construction equipment items as proposed by SUBCONTRACTOR shall be
inspected and maintained in accordance with standard codes and regulations.
8.3 If it is necessary and/or as PURCHASER’s request, the SUBCONTRACTOR shall
mobilize additional construction equipment & tools to achieve work progress at his
own cost and expenses. If the SUBCONTRACTOR cannot mobilize the additional
construction equipment, PURCHASER has the right to mobilize the construction
equipment and the cost related to the construction equipment mobilization by

PURCHASER shall be charged back to SUBCONTRACTOR.

9. SURPLUS MATERIAL AND GARBAGE DISPOSAL


9.1 The surplus/scrap material shall be returned to PURCHASER’s warehouse or
storage at the time requested by PURCHASER.
9.2 All cost related to the return surplus material and garbage disposal shall be
SUBCONTRACTOR’s responsibility.

10. SITE INSPECTION AND CONDITIONS


10.1 SUBCONTRACTOR shall make whatever arrangements that are necessary to
become familiar with all existing and anticipated conditions and matters that might
in any way affect the cost or the performance of the Work.
10.2 Any failure to duty investigates the job site, existing facilities or the foregoing
conditions shall not relieve SUBCONTRACTOR from his responsibility of
successfully performing the Work.

11. SUBCONTRACTOR’S DATA REQUIREMENTS


11.1 SUBCONTRACTOR shall provide the documents in accordance with APPENDIX-5
“Vendor Data Requirement Sheets”.
11.2 The review of SUBCONTRACTOR’s document by PURCHASER shall not relieve
SUBCONTRACTOR responsibility to meet the requirements of Service Order.
11.3 Requirement of Documents shall be as follows:
a. 1 original + 3 copies document required for quotation
b. 1 original + 6 copies for final record (1 copy in CD format)

12. DEVIATION AND EXCEPTION


Unless SUBCONTRACTOR purposes any exceptions and alternatives,
SUBCONTRACTOR’s design shall be strictly in compliance with this purchasing
specification. If SUBCONTRACTOR purposes any deviation and exception which are
deemed necessary, a list of deviations and exceptions with explanations shall be
submitted with their proposal to PURCHASER. Please fill in the FORM-2 Deviation (See
APPENDIX-5).
Only deviations submitted in writing using the FORM-2 Deviation (APPENDIX-5), and
approved by PURCHASRE/OWNER before order, will be accepted. In the event that
SUBCONTRACTOR complied without any exceptions to the requirement,
SUBCONTRACTOR shall also return FORM-2 Deviation with statement “NO

DEVIATION” printed, together with SUBCONTRACTOR’s quotation.

13. SPECIAL NOTES


13.1 SUBCONTRACTOR shall specify clearly in his proposal the code, standard and
any other procedure to complete the Work.
13.2 SUBCONTRACTOR is requested to submit their experience list of similar service,
size together with their quotation.
13.3 Preservation requirements for the equipment (if any) shall be in accordance with
Preservation Requirements (APPENDIX- 2).
