FORM FOR CONTACT ABOUT THE ARRIVAL OF DOGS AND CATS

WITHOUT COMMERCIAL PURPOSES

This contact has to be made AS SOON AS POSSIBLE (at least 48 hours before
the arrival), in order to avoid any inconvenient, namely the prohibition of
the entry of the animals due to non-compliance with the health rules laid
down.
The contact must always be in writing, preferentially by email (see contact
details in the Travellers’ Points of Entry).
The contents should at least include the information that follows:

As the owner/person authorized in writing by the owner to perform on his behalf the
circulation without commercial purposes of the (identify species), I hereby inform you
that the animal(s) that accompanies(y) me will arrive to Portugal from (identify the
country) to (identify the airport/port with a Travellers’ Point of Entry) in boat/flight
(indicate the respective data) on (indicate date) with time of arrival by (indicate the
estimated time of arrival).
The animal(s) will travel:
in the cabin: ( )
in the aircraft hold: excess of luggage ( ) air cargo ( )

In annex documentation
(Please send only the relevant pages as indicated below, thus avoiding sending unnecessary
documentation)

For all countries

Page(s) that indicate(s) the microchip number and date of placing or its reading recorded by a
clinical veterinarian;

Page(s) that indicate(s) the updated rabies vaccination.

Official health certificate issued by the country of origin

(unless it is impossible to obtain it within 48 hours before arrival)
OR
EU passport, issued before the departure of the animal, if there is no change in the sanitary
conditions recorded on it (identification - section III/rabies vaccination and revaccination -
section V).

And also for countries with a rabies risk

Pages showing rabies vaccination which led to blood test to verify the number of antibodies
(sufficient) for this disease and subsequent vaccinations until the current vaccination;

Analysis report of blood test to verify the number of antibodies (sufficient) for this disease.

Identification of the owner/authorised person on behalf of the owner and
contacts (besides name and email address, the telephone number)

The personal data collected in this form will be used exclusively to carry out all
procedures necessary for the registration of the owners/authorised persons on behalf
of the owners of non-commercial pets from countries outside the European Union at
the time of notification of their arrival.
Data processing is carried out in accordance with the General Data Protection
Regulation.
As data subject, you have the right to change inaccurate personal data and
subsequently, to object to the processing data.
I authorize the use of these data as explained above.
