Effects of Aromatherapy Menopause
Purpose: This study was a systematic review to evaluate the effects of aromatherapy on menopausal symptoms, perceived stress and depres-
sion in middle aged-women. Methods: Eight databases were searched from their inception September 8, 2015. Two reviewers independently
performed the selection of the studies, data abstraction and validations. The risk of bias was assessed using Cochrane criteria. For analysis of
the data, a meta-analysis of the studies was performed. Results: From the electronic databases, 73 articles were selected, and 19 removed
due to duplication. After two reviewers read the abstracts of 54 studies, 34 studies were selected. Complete papers for 34 original articles
were read and, 12 studies which met selection criteria were reviewed and the effects of aromatherapy on menopausal symptoms, stress and
depression analyzed using meta-analysis with RevMan. In the 2 studies which included Randomized Controlled Trials testing of aromatherapy
on menopausal symptoms and comparison of control and placebo groups were done. Aromatherapy massage was favorably effective in re-
ducing the menopausal symptoms compared to the control group (n=118, MD=-6.33; 95% CI -11.51 to -1.15), and compared to the placebo
group (n=117, MD=-4.14; 95% CI -7.63 to -0.64). Also aromatherapy was effective in reducing stress (n=72, SMD=-0.64; 95% CI -1.12 to
-0.17) and depression (n=158, MD=-5.63; 95% CI -10.04 to -1.22). Conclusion: There is limited evidence suggesting that aromatherapy for
middle-aged women may be effective in controlling menopausal symptoms, perceived stress and depression.

3) 효과크기 산출
본 연구의 효과크기는 기술된 결과 변수의 특성이 연속형 변수임
에 따라 평균과 표준편차로 분석하였다. 동일한 중재 결과를 동일한
측정도구를 사용했을 경우에는 중재군과 대조군 각 군의 최종 평균
값을 이용하여 평균차(Mean Difference [MD])를 계산하였으며, 다
양한 측정도구를 사용한 경우에는 표준화 평균차(Standardized
Mean Difference [SMD])를 계산하였다. 각각의 결과 변수의 효과
와 95% 신뢰구간(95% confidence interval, 95% CI)은 역분산(in-
verse variance) 방법을 사용하여 분석하였다.
4) 이질성 검증
이질성(Heterogeneity)이란 메타분석에서 통합하고자 하는 개별
연구간의 다양성으로 나타나는 차이를 말한다. 이질성 여부의 판단
은 메타분석의 숲 그림을 통해 연구간 신뢰구간과 효과 추정치에 공
통적인 부분을 확인하는 시각적 방법과 통계적 수치를 확인하는 방
법이 있다. 통계적 수치를 통한 검정에서 대표적인 것은 I²으로 본 연
구에서는 연구간의 이질성을 Higgins I²- statistic으로 평가하였다.
I²값이 25%인 경우 이질성이 낮다, 50% 이질성이 중간 이다, 75%

시된 연구였으며, 국내 10편, 이란 2편이었다. 연구 대상자의 연령은 증상을 측정한 연구 5편, 스트레스 5편, 우울 3편으로 나타났다.
다양하였으며, 37~65세의 중년여성이 포함되었다. 아로마테라피 유
형은 흡입방법 2편, 마사지 방법 8편, 흡입과 마사지 방법을 혼합하 2. 효과크기 추정
여 사용한 연구 1편, 흡입과 마사지 방법을 각각 사용한 연구 1편이
었다. 아로마 오일의 종류에는 라벤더가 12편 중 9편(75%)으로 가장 총 12편의 분석 논문 중에서 효과크기 분석이 가능한 5편을 대
많이 사용되었으며, 로즈제라늄, 로즈, 로즈마리, 자스민 등 16개의 상으로 아로마테라피가 중년여성의 갱년기 증상과 스트레스에 미치
아로마 오일이 단독 또는 혼합하여 사용되었다. 종속변수로는 갱년기 는 효과크기를 분석하였다(Figure 3).

갱년기 증상의 효과크기를 분석할 수 있는 연구는 RCT 2편, 간의 효과크기는 통계학적으로 유의한 차이를 보였다(Z = 2.39,
CCT 1편으로 총 3편이었으나, RCT와 CCT를 통합하여 효과크기 p = .020). 위약군 오일로 처치를 받은 대조군과 비교 결과, 갱년기
를 계산할 수 없으므로 RCT 2편에 대한 효과크기를 계산하였다. 증상은 4.14점(MD = -4.14; 95% CI -7.63 to - 0.64)의 감소 효과
연구 논문의 동질성을 확인한 결과, 사용한 도구와 실험처치에서 가 있었고, 실험군과 대조군 간의 효과크기는 통계학적으로 유의한
동질 하지 않은 것으로 나타나(aroma G versus control G: Higgins 차이를 보였다(Z = 2.32, p = .020).

스트레스의 효과크기를 계산하기 위해 이용된 스트레스 연구는 총
모형을 사용하여 갱년기 증상의 효과크기를 분석하였다. 처치를 받 5편이었으나 평균과 표준편차를 제시하지 않은 연구 2편, 결과변수
지 않은 대조군과 비교 결과, 갱년기 증상은 6.33점(MD = - 6.33; 가 동일하지 않은 연구 1편을 제외한 2편의 연구가 메타분석에 포함

결과 동질한 것으로 나타났으며(Higgins I²= 0%) 고정효과모형을 사 는 도구들로 평가하고 있었다. 먼저 갱년기 증상을 결과변수로 한 연
용하여 분석하였다. 그 결과 스트레스는 0.64점(SMD = - 0.64; 95% 구 5편을 분석하였고, 그 중 RCT 2편을 평균차를 이용하여 랜덤효과
CI -1.12 to - 0.17)의 감소효과가 있었으며, 실험군과 대조군 간의 모형을 사용하여 분석하였다. 대조 처치로 아무것도 하지 않은 연구와
효과크기는 통계학적으로 유의한 차이를 보였다(Z = 2.65, p = .008). 위약처치를 한 연구들과 비교해서 아로마테라피의 실험군의 갱년기 증
우울의 효과크기를 계산하기 위해 이용된 연구는 총 3편으로 처 상의 유의하게 감소된 것이 확인되었다. 그리고 갱년기 증상 완화를

마사지와 흡인 등 병행의 방법이 3편의 연구에서 활용되었다. 아로마 선택한 근거, 배합의 근거, 적용 방법의 근거 등을 탐구하여 보다 표준
테라피에 사용된 오일은 라벤더, 로즈제라늄, 로즈, 로즈마리, 자스 적 프로토콜을 제시할 필요가 있다.
민 등 16개의 아로마 오일이 단독 또는 혼합하여 사용되었는데 이 중 스트레스에 대한 메타 분석 결과 통계적으로 유의한 차이가 있는
라벤더가 가장 흔히 사용되었다. 본 연구에서의 결과변수인 갱년기 것으로 나타났으며, 스트레스 완화목적으로 투여된 아로마 오일은
증상, 스트레스, 우울에 대한 효과는 전체적으로 효과가 있는 것으 라벤더, 페퍼민트, 만다린, 일랑일랑 등이었다. 적용방법은 5편 중 4
로 볼 수 있다. 수세기에 걸쳐 아로마오일은 다양한 건강문제에 대해 개 연구[17-19,21]에서는 마사지를 사용하였는데 오일, 마사지 부위,
