to respectfully request for a Travel Authority for the said intended vacation as a requirement
prior to departing abroad as well as the approval for my intended days of vacation leave covering
the period December 17, 2018 to January 12, 2019. All in all, the number days of leave of
absences that I am filing is 15 days as indicated in the herein attached leave application form.
Also, may I respectfully request for a Certificate of Employment or Work Certification
covering my services rendered in the City Government of Manila to serve as attachment to my
Visa Application.
