Question 1: Burns

Ms. Ng, a 30-year-old woman, arrives at the emergency department (ED) after sustaining burn
injuries in a kitchen accident at home. She was cooking on a gas stove when her clothing caught
fire. On arrival, Ms. Ng appears anxious and is in visible distress. Emergency medical services
have already initiated initial burn care protocols during transport.

Upon assessment, it is noted that Ms. Ng has deep partial-thickness burns on her face, neck, and
both hands, covering approximately 15% of her total body surface area. The burns are
characterized by blistering and significant redness. Ms. Ng reports intense pain despite the
administration of analgesics during transport.

The triage nurse quickly identifies the need for a multidisciplinary approach. The attending
emergency physician assesses the burns and orders appropriate interventions. The nursing staff
initiates fluid resuscitation following the Parkland Formula, considering the depth and extent of
the burns. Her vital signs were as follows:

• Temperature : 36.5’C
• Pulse rate : 117 beats/minute
• Respiratory rate : 36 breaths/minute
• Blood pressure : 98/52 mmHg
• Sp02 on room air : 91%
• Pain score : 7/10 (Numerical Pain Scale)
• Arterial blood result : Hypercapnia

a. A burn patient's fluid balance is closely monitored during the emergent phase. 10 marks
Analyze the nursing assessment criteria for fluid resuscitation, considering
factors such as urine output, vital signs, and laboratory values.

b. A burn survivor is experiencing symptoms of post-traumatic stress disorder 10 marks


(PTSD). Analyze the psychological impact of burns on mental health and the
role of the nurse in providing holistic care. Develop a nursing care plan that
addresses the psychological needs of burn survivors throughout the
rehabilitation process.

Question 2: Infectious Disease

Mary, a 25-year-old patient has just been diagnosed with a sexually transmitted disease (STD)
after presenting with symptoms of discomfort, pain when peeing, unusual vaginal bleeding and
itchy at genitals area. Concerned about her symptoms, she decided to seek medical attention. After
a thorough examination and a series of tests, Mary received the unexpected news—she had been
diagnosed with a Sexually Transmitted Disease (STD).

Upon hearing the diagnosis, Mary's emotions ranged from shock to distress. The healthcare team,
including her primary care nurse, was quick to step in and provide support. Mary was visibly upset,
her mind racing with questions about the implications of the diagnosis on her health, relationships,
and daily life. The healthcare team, including the nurse, is tasked with providing comprehensive
care to the patient.
