INDEMNITY BOND

This Deed of Indemnity Bond is made at Delhi, on this 01 ST DAY OF SEPTEMBER 2014, by
SHRIMATI ANISHA BEGUM WIFE OF SHRI MUSHTAQ KHAN RESIDENT OF H.NO. D-184,
NW SEELAMPUR, DELHI-53, hereinafter called the Executant IN FAVOUR OF President of
India,/Ration Card Authority, hereinafter called the Ration Card Authority.
And Whereas the Executant is the holder of a Ration Card vide Ration Card No. APL-65101726,
in the name of my husband MUSHTAQ KHAN at my said address.
1. That the said Ration Card has been lost by me any where & I have lodge the
information/FIR with the Police Station, at _________________________, Delhi-53.
2. That I am applying for duplicate Ration Card to the Authority, and I shall not misuse if
found the said lost Ration Card & return to the said Deptt.
3. That my family income from all sources is Rs. 48,000/- per annum.
4. That if in future any dispute arise for the same then I will be fully responsible.
In witnesses whereof the Executant have signed this Deed made at Delhi on this day,
