PHYSICAL EXAMINATION
Examined an adult male, ambulatory, with normal gait, cooperative and oriented as to
time, place and person.
1. Multiple abrasion wounds at right elbow, reddish in color.
2. Multiple abrasion wounds at right lateral flank area and right upper back, lateral area,
reddish in color.
3. Hematoma at mid-upper back, lateral area, purplish red in color.

CONCLUSION:
Physical injury noted on the person Christian Hope Malayo Matutis, said injury,
barring complications with present condition but not clinically apparent during the time of
examination, may cause incapacity for daily activities of the injured for less than 5 days.
