Instructions: Please complete this form as it pertains to your endorsement request for inclusion in Reggae Month| its
Kingston for February. It will serve as a key element in the evaluation of your proposal; along with the application form,
an event/partnership summary of no more than two (2) pages should be attached. Please note that all requests for
endorsement should meet the following basic criteria:
1. the event must fall between February 1 – 28, 2015
2. the event must be held in the Kingston and St. Andrew Metropolitan Region
3. the proposed activity should be Reggae related

The completed application form must be submitted to the Ministry of Tourism and Entertainment,
64 Knutsford Boulevard, Kingston 5 by Wednesday September 10, 2014 by 3:00p.m. Only typed applications will be
accepted.

* Entities who desire to partner with Reggae Month| its Kingston for February should complete only Part 1 and Part 2
of the application form AND submit a partnership proposal of no more than two pages to the Ministry of Tourism &
Entertainment.

Response Time: We will respond to you within fourteen (14) business days after receipt of your proposal.
