If you are unable to attend this appointment, please notify the appointments office by
telephoning (01302) 642500 between the hours of 8.00am and 4.00pm Monday to Friday. If
you have a transport query please call 01302 644592 ( answer phone service available ) or
01302 644593. This office is open Monday to Friday 8am-6pm.

If you fail to attend it will be assumed that you no longer require an outpatient appointment.
You will be removed from the waiting list and your doctor informed.
