CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This contract of service entered into and executed this 25th day of May, 2024
at Brgy. Lamot 2 Calauan, Laguna, Philippines by and between:

Sangguniang Barangay of Lamot – 2 with office address at Purok 2 Pili Drive Street Barangay Lamot 2,
Calauan, Laguna, herein represented by HON. MAC JEFFERSON T. ROXAS, hereinafter referred to as
the First Party;

and

JESTONI RITUAL ASPIRAS, of legal age, resident of Paete Laguna, the owner of STA. CECILIA
BANDA 3 hereinafter referred to as the Second Party;

WITNESSETH

That pursuant to the stipulations in this contract, set forth by the Sangguniang Barangay of Lamot 2 and
Mosiko, the following terms and conditions for the Second Party are hereby set:

TERMS AND CONDITIONS:

1. SCOPE OF SERVICE: The Mosiko agrees to provide cultural performances, including music, dance, and
other artistic expressions, for the Cultural Activities held on May 27, 2024, in Barangay Lamot 2.

2. PERFORMANCE DETAILS: The Mosiko shall perform cultural activities as agreed upon with Barangay
Lamot 2, including the duration of performances, specific acts, and any special requirements for the event.

3. COMPENSATION: In consideration for the services rendered, Barangay Lamot 2 agrees to pay in CASH
the Mosiko, the sum of Twenty-two thousand pesos [22,000.00] for their performance on May 27, 2024.
4. REHEARSALS: The Mosiko may be required to attend rehearsals prior to the event to ensure the
smooth execution of the cultural activities.

6. CANCELLATION: In the event of unforeseen circumstances leading to the cancellation of the event, both
parties agree to negotiate in good faith regarding any compensation or rescheduling of services.

7. CONFIDENTIALITY: Both parties agree to maintain the confidentiality of any information shared during
the course of this agreement.

8. GOVERNING LAW: This Contract of Service shall be governed by the laws of the Philippines.
IN WITNESS WHEREOF, both parties have hereunto set their hand this 25th day of May, 2024 at Brgy.
