AFFIDAVIT

I, MERCEDES M. SANTE, married, of legal age, Filipino citizen, and presently

residing at 6 Aguilar Street, Barangay Bungad, Project 7, Quezon City, after having been duly
sworn to in accordance with law, do hereby depose and say:

1. That I am the Attorney-in-Fact of my son, ROMAN M. SANTE, the registered owner of owner
of MANCY INTERNET CAFE, with business address at 6 Aguilar Street, Barangay Bungad,
Project 7, Quezon City.

2. That the Original/Duplicate copies/receipts of the Original Certificate of Registration (COR),

Unused Receipts and Invoices, Original Latest to Print (ATP) were misplaced in my possession,
and were lost and I took pains to look for said receipts/records but to no avail and totally lost.

3. That I am executing this statement to attest to all the above facts, and for closure on my business.
