Dear Ma’am/ Sir,

I am writing to express my intentions of asking your permission to conduct a

Home Visitation to the Learners of Pagbilao Central Elementary School. The month
of June are the Vacation of the Learners and as part of our Program “Kubol Kubol” and
in accordance to “No Child Left Behind” we will conduct a Home Visitation with a
purpose of Reading Assistance and Enhancement in English and Filipino along
with Numerical Enhancement to each Struggling Learners of the said school.

The Community Learning Space was prepare exactly within their Home Premises either
Inside or Outside of the House, the said Activity will also provide a Consent Form to the
Parents/Guardian of the Learners noting that they agree upon conducting the Reading
and Numerical Enhancement to the learners.

The said Activity will Start by June 3 (Monday) and will continue till the end of the
Vacation, Home Visitation will only be conducted during weekdays and willingly agreed
along with other Teacher Aides.
