1. That I, is the sole proprietor of SFZ AGRI TRADING with business address at Zone I,
Lumbangan, Zamboanga City;
2. That I have decided to retire the business Contractor-Rental Service starting May 29,
2023 due to financial constraints.
3. That affiant is executing this affidavit for the purpose of effecting the necessary
amendments in the renewal of business permits, other government business requirements, and
other pertinent documents;
4. That affiant is executing this affidavit for the attestation of the veracity and truthfulness of
the foregoing statements.

This Contract is entered into by and between FERNANDITO AGUILA ONA (LESSOR), of
legal age, Filipino, married and a resident of Gov. Camins Ave., Canelar, Zamboanga City,
Philipppines, and LILAMAY UNDADI SURBITO (LESSEE), also of legal age,Filipino, single and a
resident of Lumbangan, Zamboanga City, Philippines,whereby the LESSOR hereby lease to the lesee,
subject to the following terms and conditions:
1. That the premises leased have been received by the LESSEE in good order and condition;
2. That the lease covers One Commercial space located at Gov. Camins Ave., Canelar,
Zamboanga City;
3. That the term of the lease shall be for a period of Two ( 2 ) years beginning March 01, 2014
and will end on March 01, 2014;
4. That the monthly rental of the leased premises shall be Two Thousand Pesos ( P 2,000.00 )
per month, payable in advance within the first five ( 5 ) days of the Month;
5. That all the ordinary repairs within the premises that arise from daily use of the premises shall
be for the sole account and expense of the LESSEE, without right of reimbursement;
6. That the LESSEE shall used the leased premises exclusively for business purposes not for
family dwelling or any other purposes;
7. That the LESSEE is expressly prohibited to subject the leased premises to any one without the
express consent of the LESSOR in writing;
8. That the Charges for water and light shall be at the sole account of the LESSEE;
9. That the LESSEE shall notify the LESSOR at least thirty ( 30 ) days in advance , Should he
decide to abandon the leased premises;
10. That the violation of any of the above terms and condition will produce the rescission of the
contract of lease.
