Brief Description of Incident: A flash over incident happened on 13/06/2020 at 07.15 PM near switch
Yard. The 3Phase 440 Volts, temporary power cable to storm water reservoir pumps from SS#1
(PCC 1301) might have damaged and got minor punctures at the half way due to the nearby site
grading/paving works and resulted in a flash over, when the rain water ingressed in to the already
damaged cable.

Probable cause of Incident: Electrical protection devices are not functioning/tripped in the source
location.
The cable laid above the ground and further damages during the grading and pavement works

Suggested corrective action: Cable shall be routed properly as per the Standards/Specification.
Inspection shall be done on regular basis and ensure that the cables as well as the protection devices
are defect free.
Electrical permit system shall be followed religiously.
