3. Incident Description

 Type of Incident: injury and near miss at Left hand Palm

 Description of Incident: While working at 4th floor roof shuttering, the
completed sheet covering and those gaps are filling with wood materials,
while cutting the wood the machine handled wrongly by labour, because of
this accident happened. And only the particular labour is involved.

 Were there any injuries? Yes

 Labour was injured with left hand thumb.
 Were there any damages? Yes
 Left Thumb got cut

 Witness Statements: I was present at site and we were working at 4 th floor
corridor portion roof shuttering work, while gap filling with wood work, at the
time of cutting wood the accident was happened.
6. Actions Taken

 Immediate Actions Taken: First aid made immediately at site and took him
to nearby hospital and they suggested to take for good facility hospital.
 Follow-Up Actions Planned: Took him to People Tree Hospitals and
admitted.
