AFFIDAVIT OF LOSS

I, JASSMINE LOBRIGO LOBIO, Filipino, of legal age, single and with postal address at
4023- 284 Laguerta St. San Vicente San Pedro Laguna, after having been duly sworn to in
accordance with law, do hereby depose and say that:

1. I am the account owner of the Gash account number 09167381844

2. On, December 16, 2022, my phone was stolen wherein my sim card is in the phone.

3. I looked and tried to look for the phone but unfortunately, I never found

4. I have no longer access to my gash account.

5. I am requesting to transfer all of my remaining funds to my new gash

6. I now consider the said sim card as lost and I am executing this affidavit for whatever other

legal purpose it may serve;

7. I am executing this Affidavit to attest the truthfulness of all the foregoing facts.
