Ladders the proper height for the work at hand
Ladders are free of visible defects
Workers do not overextend reach of the ladder
Ladders are placed on a solid level surface
A-framed ladders used in the open position
Nonconductive ladders are used when necessary
Workers not on top two steps of A-framed ladders
Straight ladders secured and extend 3’ above the landing
Fall Protection
Floor opening covers are properly secured and marked
Shafts and floor openings properly guarded
Guardrails top/mid rails and toe board in
good condition
Full body harnesses are properly used as fall protection
Lanyard of proper length to limit falls to less than 6 feet
Lanyard secured to proper anchorage
Lifelines secured to proper independent anchorage
Workers utilizing the proper fall protection
Scaffolds
” Competent person” on site
10”x10” mud sills in place solid footing
