approved as B Code

- PTW issuers and receivers should get appropriate training from an authorized third party or
competent person.
- Receivers shall keep permit copies inside the permit box at the job location, always, so that it is
available for inspection or cancellation if necessary.

1. Scope and Objectives


This standard establishes the minimum safety requirements and responsibilities for the
issuing and receiving of Safe Work Permits and specifies the requirements for certifying
those responsible for implementing the process /individual process. Units may develop
additional practices or procedures to cover their own specific requirements. When Issued
Work Permits serve as official records of conditions and requirements agreed upon between
the Permit Issuer and Permit Receiver. Work Permit listed the minimum safety precautions
to be taken and the hazards which must be controlled.

2. Purpose
The purpose of the Permit to work OH&S Requirement is to outline the formal
administrative process to ensure the control of high-risk activities by implementing a
systematic process used to authorize controlled work in nonstandard, potentially hazardous
conditions. The process includes risk assessment, the establishment of safety protocol
specified in the Permit to work, communication, and oversight to minimize the risk to carry
out the work under strict controls.

This standard applies to the work being done by all personnel on the construction site.

The Permit to Work Policy is split into several components dependent on the type of work
to be performed, Cold Work, Hot Work, Working at Heights, Excavation work and/or
working in Confined Spaces, Lifting Permit, Night Work Permit, and the safety
requirements of each.

3. Intent
x The Salco Internal Work Permit Procedure establishes minimum guidelines and responsibilities for the
Issuing/receiving of work permits, and for certifying those responsible for implementing the system.
When issued, permits serve as an official recordof conditions and requirements agreed upon by the issuer
and receiver. They list minimumsafety precautions to be taken and hazards which must be controlled.
Salco undertakes to comply with to and incorporate all permit-to-work procedural standards and
guidelines as specified in the DD-SWD-SW-SWD-000-000-DGD-MNR-HS-000021.

5. Definitions
Restricted areas: - Those areas or activities which have been designated Salco as requiring
the work permit procedure.

Authorized Person (Work permit issuers): - Employees designated by the issuer, who
must be experienced, competent and familiar with the requirements of work permit system
and be able to perform the duties associated with issuing work permits. Designated
representatives can only sign work permits.

Permit Holder (Work permit receivers): - supervisors, Foreman, or other company


employees who have been designated by their Construction Manager to sign and receive
work permits (also referred to as receivers).

6. PTW Training.
Safety department will assign one designated Trainer for the course. In case
non-availability o f Trainer, safety manager/ safety supervisor or officer will conduct.
the trainer.
x All personnel involved i n the use of the Permit to Work system or Permit procedures
shall be both trained and assessed as competent to the appropriate level.
x Refresh training shall be conducted every one year of period.
x Record kept at Safety office of Training.

7. Function of Work Permit System:


A permit to work (PTW) system is a formal recorded process used to control work which
is identified as potentially hazardous. It is also a means of communication between
site/installation management, plant supervisors and operators and those who carry out the
hazardous work. Essential features of PTW systems include:

The work permit system authorizes specific construction, maintenance, inspection, or


repair to be conducted in the Construction areas. The system is intended to be applied to
those jobs which represent a potential hazard to facilities, personnel, or equipment.

Work permit forms must be issued, as appropriate for specific work on a specific site. These
work permit forms are for all types of work in restricted areas. Work must be identified as
one or a combination of the following and the proper permits issued.

8. Type of Permits.
Cold Work Permit - Work that does not create enough heat or sparks to ignite flammable
gas-air mixtures or flammable materials and such work is not done in confined space area.
In classified areas, cold work will primarily be work done with hand-held, hand operated
tools, which do not normally produce sparks.

Hot work: - Any work that develops sparks, flames, or heat sufficient to cause ignition.
This work permit will apply to the use of spark or flame producing tools and equipment,
including the use of internal combustion engines in restricted areas.

Working at Heights: - Any work that has a fall exposure of 1.8 meters above or greater
is considered as working at heights.

Confined space: - C.S Entry is the entry of personnel into any space or structure (i.e.,
tank, vessel, vault, excavations when 1.2 m (4ft) or deeper, etc.) not normally intended for
human occupancy, where entry, movement within, or exit is restricted.

Excavation Work: - Work to be performed by making a depression in the Earth’s surface


formed by Earth removal which produces unsupported Earth conditions by reason of the
excavation work.

Lifting Work: - Includes any equipment used at work for lifting or lowering loads including
attachments used for anchoring, fixing, or supporting it. The regulations coverthe wide range of
equipment including cranes, forklift trucks, lifts, hoist, mobile elevating work platforms and
vehicle inspection platform hoist the definition also includes lifting accessories such as chains,
slings, eyebolts etc.

Night Work Permit: -Work activities during the hours of darkness are greatly
discouraged due to the associated hazards and will only be allowed by fulfilling
requirements under the valid night work permit.

9. Permit-to-Work Procedure

Permits-to-work provide a formal safety control system aimed at prevention of accidents, damage to
property and damage to products, when for see ably hazardous works are undertaken. International
and national standards and client safety standards placed further duties on employers to “provide and
maintain plant and systems of work that are, so far as reasonably practicable, safe and without risks
to health”.

The permit to work consists of a document which.

ƒ Details of the work to be done.


ƒ Details of the precautions to be taken.
ƒ States that all foreseeable hazards have been noted.
ƒ States the control measures to be implemented.

Permits to work do not, in themselves, make a job free from risk: they rely upon effective control
and coordination in order that hazards are identified, and risks are suitably and sufficiently assessed.

10. Requirements of a Permit to Work.

11. Roles and responsibilities.


The procedure shall identify clear roles and responsibilities for all key stakeholders within the
procedure. As a minimum the following roles and responsibilities shall be identified and allocated to
individuals within the organization. Each of the roles identified shall be independent and cannot be
held by the same person. Each role shall be allocated to named individuals and shall be done
formally in writing.

Authorized Person (Permit issuer)


The Authorized Person (AP) shall be given the authority to issue and sign permits on behalf of the
employer. It is essential that the AP has appropriate knowledge about the hazards associated with
the relevant activity to allow them to identify those hazards and control measures (e.g. isolations)
correctly. Including:

x Shall ensure that all foreseeable hazards associated with the proposed job have been
identified and appropriately assessed.
x Shall ensure that all steps necessary to ensure the safety of the site or installation have been
identified.
x Shall ensure the work site has been examined, and all precautions specified to be taken
before work commences (including isolations) have in fact been taken and shall remain
effective while the permit remains in force.
x Shall verify that the permit holder, including further staff tasked with undertaking works
under the PTW, have appropriate competence to undertake the role and fully understand the
risks.
x Shall ensure that the permit holder is aware of the precautions taken, any additional ones
which are to be taken, particular equipment to be used or worn, and any other procedures
which are to be followed.
x Shall ensure that any work activities that may work activities that may conflict with one
another are clearly identified and conflictions are avoided, or precautions included on the
permit (e.g. Use of welding shields)
x Shall ensure that all relevant people are aware of the permit’s duration, and action to be
taken if the work is suspended; (viii) Shall authorizing, by signature, the work to proceed
following confirmation that all control measures are implemented and have been signed
off.
x Shall ensure that copies of all issued permits are displayed at an appropriate location and in
a consistent arrangement so that site personnel can readily see and check which equipment
is under maintenance and not available for operation.
x Shall undertake ongoing inspections of the PTW to ensure risk control measures are being
maintained.
x Shall ensure that the work site is examined at any time when work is suspended and before
it is restarted, and finally when the work is completed to ensure that it is in a safe condition.
x Shall ensure that the shift handover procedure is complied with shall ensure that the
worksite is examined on completion of the works and the PTW is correctly closed, and the
relevant persons informed.
x Shall ensure appropriate records of all permits and the master controls sheet are maintained.

Permit Holder (Permit receiver).


The Permit Holder is the competent person who has requested the permit to be issued and shall be
fully responsible for the works whilst they are being undertaken. The permit holder and AP (permit

issuer) shall not be the same person including: -

x Shall ensure that they and the people working with them understand the operation of (and the
consequences of non-compliance with) the PTW systems applicable to the areas in which they
are responsible for work.
x Shall ensure any necessary information, instruction or training is given to users to ensure that
they understand the PTW systems, and the specific precautions required for their work.
x Shall ensure that the AP (permit issuer) and permit users fully understand their responsibilities
under the PTW system.
x Shall ensure that the conditions and precautions specified in the permits are fully understood,
implemented, and effectively monitored.
x Shall immediately stop / suspend work if conditions require or if the PTW requires change.
x Shall communicate effectively with all parties during the operation of the Permit.
x Shall ensure that all parties are aware of the completion of the works and the permit is correctly
closed with the Authorized person (AP)

Work Party (Holder)


The Work Party who undertakes the work shall always comply with the requirements of the PTW
including DD-SWD-SW-SWD-000-000-DGD-MNR-HS-000021.

x Shall understand the scope of work and the control measures implemented.
x Shall participate in the development or review of the risk assessment.
x Shall ensure that all control measures defined in the PTW are strictly followed.
x Shall not interfere with the work of their colleagues and shall adhere to the roles and
responsibilities assigned under the PTW.
x Shall cease work and inform the Permit Holder when there is a breach of the PTW conditions;
and
x Shall leave a clean and safe work site.

12. Specific Permit-to-Work: Confined Spaces Introduction:


A confined space means any place, including any chamber, tank, vat, silo, pit, trench, pipe, sewer, well or
other similar space in which, by virtue of its enclosed nature, there arises a reasonably specified risk. No
person whether working for Salco, or working in the contract project, shall enter, or be instructed to enter a
confined space for any purpose:
ƒ Unless it is not reasonably practicable to achieve that purpose without such entry.
ƒ Until they have received a valid permit to work.

12.1 Competent Person Duties:

Prior to raising a permit-to-work, the Competent Person shall:


x Ensure that the requirements of the company’s procedures for the management contractors
have been met.
x Ensure that a permit to work is issued as per company procedures.
x Ensure that the works are adequately monitored for safety compliance.
x Ensure Permits are closed as per company procedure.

Emergency equipment and procedures including: -


x Rescue harness.
x Lifting tripod or other suitable means to retrieve persons from confined space.
x Emergency breathing apparatus.
x First aid kit.

x An observer and means of communication.

13. Specific Permit-to-Work: Excavations


Introduction:
All grounds disturbance works on parks and trails projects shall be subject to a Permit-to-
Excavate. Excavation Permits shall only be issued by a Competent Person. Excavations shall be
carried out in accordance with all relevant legislation.

Competent Person Duties:

The Competent Person shall be required to ensure that:


x Contractors have sufficient information, where available, regarding known positions of cables
and underground services to plan the excavation works safely.
x Contractors have sufficient information, where available, regarding ground conditions and
stability of adjacent structures to enable a safe means of excavation to be selected.
x Where excavations could compromise the stability of adjacent structures, a suitably competent
Design Engineer is consulted to review the works to be undertaken.
x Any other information held by the company which may be relevant to the chosen method of
excavation selected by the contractor is made available.
x Ensure that the requirements of the company’s procedures for the management of premises
contractors have been met.
x Ensure that a permit to work is issued as per company procedures.
x Ensure that the works are adequately monitored for safety compliance.
x Ensure Permits are closed as per company procedures.

14. Specific Permit-to-Work: Hot Works


Introduction:

Hot work is defined as works that involves temperatures that could give rise to risks of fire and
ignition of flammable substances and combustible materials and includes work which involves the
use of naked flames or generation of sparks, smoke or fumes.

The following works shall be subject to a permit-to-work (Hot Works)


x Brazing and soldering.
x Bitumen boilers.
x Electric arc welding.
x Gas welding or cutting.
x Hot air welding.
x Disk cutting in confined spaces and areas where there is a high risk of fire or explosion.

Competent Persons Duties:

The Competent Persons (Hot Works) shall be required to ensure that:


x A permit-to-work is issued as per the procedures in this manual.
x The works are adequately monitored for safety compliance.
x Permits are closed as per the procedures in this manual.

Pre-Start Inspection:

Before signing the permit-to-work, the Competent Person shall inspect the working area for
the following control measures (where applicable):
x Cutting and welding equipment is in good repair.
x All gas hoses are securely clipped.
x Welding / Cutting equipment is fitted with regulators and flashback arrestors.
x The area is adequately ventilated.
x Gas cylinders are secured in a vertical position on a trolley.

Precautions within 10m of works (where practicable):

x Floors swept clean of combustibles;


x Combustible floors have been damped down or covered with sand or fire-resistive sheeting.
x The explosive atmosphere in the area has been eliminated.
x Flammable liquids have been removed, or if removal is impracticable, protected with fire-
resistive tarpaulins or metal shields.
x All wall and floor openings are covered.
x Fire-resistive tarpaulins are suspended below work.

Works on Walls or ceilings:


x Construction is non-combustible and without combustible covering or insulation.
x Combustibles have been removed from the other side of the wall.

Work on enclosed equipment:


x A permit-to-work (Confined Spaces) has been issued by a suitably Competent Person.

Works on Roofs: (Bitumen boilers, lead heaters etc.)


x Gas cylinders are sited at least 3m from the burner.
x If sited on roof, heat insulating base provided.

Fire watch:

The Competent Person shall also ensure that the following general controls are in place:
x Suitable and sufficient fire extinguishers are readily available.
x Operatives are trained in the use of fire extinguishers.
x Adjacent smoke heads are covered to prevent false alarms.
x Security & Reception are advised if it is deemed necessary to take the local alarm system
off automatic 911 call out.
x All fire doors are closed.
x Arrangements are in place to make sure that the hot works area is not left unattended, for
example during tea breaks, lunch etc.
x Operatives are aware of actions in case of emergency.
x No other people are working in the area that might be endangered by the hot works.
x Arrangements are in place to carry out a check of the area at least 1 hour after the hot works
have ceased.
x Security is advised that the alarm should be put back on automatic 911 call out at cessation
of the work.

15. Method statements:


Hot works covers a multitude of operations. In general, the Competent Person shall ensure that the
control measures listed above are accounted for, where applicable, within the contractors’ method
statement. The following elements will however be common to most method statements:

x Name and address of the contractor involved.


x Name and address of the site to which the method statement relates.
x Names of the supervisor/foreman and appointed safety adviser and arrangements for
monitoring the work.
x Description of the work to be carried out, including type of system / components.
x Details of the personnel to be used and their training / experience.
x The steps to be taken to avoid the risk of explosion and in particular all gas testing that is
required.
x All personal safety equipment is required.
x The action to be taken to segregate the work area and post warning notices.
x Actions to be taken in emergency situations.

16. Specific Permit-to-Work: Electrical Services


Introduction:

The following works carried out on electrical equipment located on the Company’s premises shall
be subject to a permit-to-work:

x Switching off any isolate, switch fuse, distribution board, or mains circuit board that may
affect the company’s information technology systems or the safety of anyone on the
company’s premises.
x Test on live electrical apparatus.
x Test on electrical distribution systems that need the installed safety systems/barriers
defeated or removed.
x Work on electrical distribution systems that expose personnel to shock hazards.
x Work on remote and automatically controlled low voltage switchgear.
x Isolation of electrical equipment to make it safe unless carried out by a competent person
holding a valid Authorization Certificate.
x Test on any earthling system whilst the supply is still live.

Electrical equipment is defined as anything used, intended to be used or installed for use, to
generate, provide, transmit, transform, rectify, convert, conduct, distribute, control, store, measure
or use electrical energy.
Live: means that the equipment in question is at a voltage, by being connected to a source of
electricity as, for example, in normal use.

Charged: means that the item has acquired a charge either because it is live or because it has
become charged by other means such as by static or induction charging or has retained or regained
a charge due to capacitance effects even though it may be disconnected from the rest of the system.

Live work is defined as work on or near conductors that are accessible, and which are 'live' or
charged’. Dead means not electrically 'live' or 'charged'.

Disconnected is used to describe equipment, or a part of an electrical system, which is not


connected to any source of electrical energy.

Isolated is used to indicate equipment, or part of an electrical system, which is disconnected and
separated by a safe distance (the isolating gap) from all sources of electrical energy in such a way
that the disconnection is secure, and cannot be re-energized accidentally or inadvertently.

Low voltage is regarded as a voltage exceeding 50v AC or 120v DC between conductors or earth,
but not exceeding 1000v AC or 1500v DC between conductors or 600v AC or 900v DC between
any conductor and earth.

Competent Person Duties:

The Competent Persons shall be required to:


x Ensure that the requirements of the company’s procedures for the management of premises
contractors have been met.
x Ensure that a permit-to-work is issued as per the procedures in this manual;
x Ensure that the works are adequately monitored for safety compliance;
x Ensure Permits are closed as per the general permit-to-work Procedures.
x Ensure that there is adequate working space, adequate means of access and adequate
lighting.

Model Method Statements:


x Method statements should normally include the following so far as is relevant in the
circumstances:
x Name and address of the contractor involved.
x Name and address of the site to which the method statement relates.
x Names of the supervisor/foreman and appointed safety adviser and arrangements for
monitoring the work;
x Description of the work to be carried out, including type of system / components;
x Details of the personnel to be used and their training / experience;
x All personal safety equipment that is required;
x The action to be taken to segregate the work area and post warning notices;
x Actions to be taken in emergency situations;
x Any inspection, examination, testing and commissioning required on completion;
x The action required to return the low voltage electrical equipment to service;

Method Statement for working on dead electrical equipment:

In addition to the above contents, the following should be included, where applicable:
x The necessary steps to be taken to protect against inadvertent contact with other live parts
nearby. Wherever practicable, this should be achieved by the erection of physical barriers
and/or the use of temporary insulation.
x The arrangements for identifying electrical equipment before disconnection or isolation.
Wherever possible this process should be aided by the use of appropriate drawings, diagrams
and other written information. Labeling on circuits and equipment may be used to assist in the
identification process, it must however never be assumed that labeling is correct;
x The arrangements for ensuring that once the circuit or equipment to be worked on or near has
been identified, that it shall be disconnected from every source of electrical energy;
x The arrangements for ensuring that electrical equipment, which has been made dead, cannot
become electrically charged during the work. Wherever practicable this should be carried out
by locking off all isolators. Where such facilities are not available, the removal of fuses or links
is permissible.
x The arrangements for testing that the isolated circuit or equipment including all parts to be
worked on, or near, are dead, even if the isolation has been achieved automatically through an
interlocking system. If it is a three-phase system or equipment with more than one supply,
prove that all supply conductors are dead. The device used for proving dead shall itself be
proved immediately before and after testing;

x The arrangements to ensure that, even if the above precautions fail, all conductors are earthed
using properly designed earthling devices or earthling leads
x Where applicable, those additional earths are available at the point of work, if this is remote
from the point of isolation. But these should be applied only after proving dead at the point of
work. (These procedures are essential for high voltage apparatus and stored energy equipment.
The earthling conductors and their connections should be suitable for the energy that may flow
in the event of a failure of the above precautions.

Note: Earthing low voltage equipment is particularly desirable if there is a risk of re-
energization. In other low voltage equipment, however, it may be physically impractical to
apply earths, or the risk of short circuit from introducing an earth near adjacent live parts may
outweigh the benefit of earthling the apparatus being worked on.

Works on, or near, Live Electrical Systems:

Work on or near live conductors shall only be permitted in exceptional circumstances and only
when directly authorized by the Manager or the Engineer having taken prior advice from an
authorized or competent person.
Live working shall not be permitted unless each of the following is true:
o It is unreasonable for the work to be carried out dead;
o Suitable precautions can be taken to prevent injury; and
o It is reasonable in all circumstances to work live:

The decision to work live must not be taken lightly. All economic and operational factors must be
evaluated against the risks involved before planning.

17. Permit-to-Work: Work at Height


Work at height means work in any place, including a place at or below ground level or when a
person is accessing or exiting from such a place (except via a staircase in a permanent workplace)
where if regulatory measures are not taken, a person could fall a distance likely to cause personal
injury.
A risk assessment, safe working procedure/method statement and permit to work covering all work
must be authorized and in operation for the duration of the task Unless it is not reasonably
practicable to achieve that purpose without working at height.

Until they have received a valid permit to work.

x Work at Height Permits-to-Work shall only be issued by a Competent Person.


x Note: Work at Height Permits-to- Work confer a right to work at height; they do not confer
a right to carry out works that are subject to other Permits-to-Work. e.g. asbestos removal
in a confined space would require two Permits-to-Work, one to work at height, the other to
carry out the asbestos removal.

Competent Person Duties:

Prior to raising a permit-to-work, the Competent Person shall:


x Ensure that the requirements of the company’s procedures for the management contractors
have been met.
x Ensure that a permit to work is issued as per company procedures.
x Ensure that the works are adequately monitored for safety compliance.
x Ensure Permits are closed as per company procedures.

Work at Height method statements should normally include the following so far as is relevant in the
circumstances:

x Description of the work to be carried out.
x Details of the personnel to be used and their training / experience.

x Details of the possible hazards, including environmental.
x Details of the arrangements for access to and egress from the work area.
x Details of the PPE to be used for the protection of the persons working.
x Details of measures to reduce the risk of personal injury by a falling object or person.
x Details of any work on or near to a fragile surface and measure taken to minimize any risks.

18. Issuance and Approval of Work Permit Forms

x The receiver (Supervisor, General Foreman) requests the work permit forms from the
Safety Department.
x The issuer will issue the work permit after he (or his designated representative) has
conducted a joint site inspection with the receiver as appropriate, performed the
necessary gas tests, (If required) listed applicable precautions and restrictions, and
reviewed aspects of the job with the receiver.
x Before issuing the work permit all necessary actions to approve the proposed work
and/or possible configuration changes must be completed.
x The issuer and receiver are jointly responsible for the safety of personnel and
equipment in the area. The issuer is responsible for specifying precautions so that the
work can be done without incident. The receiver is responsible for performing the
work in accordance with the conditions and requirements agreed upon with the issuer
and written on the work permit.
x For some jobs, the issuer (or his designated representative) may determine that a joint
site inspection is not required for jobs with extremely low risk. Examples include: cold
work in a safe area, carpentry, lubricating, cleaning, brush painting, non-flash
photography, instrument checking and a vehicle entering a non-hazardous area.
x When there is any doubt about whether a hazard exists, a joint site inspection must be
conducted. When the issuer determines a joint site inspection is not required, he must
specify the reasons in the appropriate space on the permit. Renewal of the workpermit
is permissible for two consecutive operating shifts. The renewal requires the
signatures of both the issuer and the receiver coming on shift.
x Signing the work permit indicates complete agreement with the conditions stated on
the form and transfers the responsibility to those coming on shift.
x The work permit is for specific work on a specific site. It cannot be transferred except
to the following shift. Transferring from one craft to a different craft is not permitted.
x A work permit may be issued for periods over 12 hours but not more than 7 days. This
applies to extended construction or maintenance work where the job requirements
remain essentially unchanged. The permit must be approved by the project manager
or above of the issuing department and the following conditions must be satisfied:
x It must be highly unlikely that conditions will develop which would create an
additional hazard to personnel or operating facilities.

x Precautions must be taken prior to starting the job, to ensure safe conditions
throughout the job.
x Both the project manager and the construction manager must be in agreement as to
job conditions and risks involved and both must sign the work permit.
x The receiver of a work permit must keep in his possession or within view of the job
site for the duration of the job so that it may be available for inspection and
cancellation if necessary. If he must leave the job site for any reason, for example to
obtain additional tools or equipment, etc., he must, if the issuer concurs, relinquish
supervisory control to a senior crew member who is fluent in written and spoken
English, until he returns. The senior crew member must know the requirements of the
job and understand the precautions/conditions stated on the work permit.

19. Right to Stop Work and Cancel Permits Work permits:


x Shall be closed by both the issuer and the receiver except where distance and
remoteness make signing impractical, an alternative closing method is determined
and stated on the work permit when it is issued.
x The issuer may stop a job and withdraw a work permit if he feels that the Safety of
the job does not meet the conditions specified on the work permit.
x When work is stopped the issuer must pick up the work permit, write on the permit
the reasons for stopping the work, the time, and other relevant information.
x When conditions are safe again the issuer may issue a new work permit.

The receiver:

x Has the responsibility to stop work and advise the issuer of his action any time he
determines that the conditions of the job do not meet the conditions on the work
permit. He and the issuer shall take action to correct deficiencies and resume the job.
x All unnecessary personnel whose presence adversely affects the safety of the job
must be kept away from the job site or work shall be stopped.

20. Closing Work Permits


Work permits: - Shall be closed by both the issuer and the receiver except wheredistance
and remoteness make signing impractical, an alternative closing method is determined and
stated on the work permit when it is issued.

21. Filing Work Permits


The issuer shall keep the original copy of the work permit or permit for a minimum of
three (3) months.

22. Certification of Issuers and Receivers of Work Permits

x Work permit courses will be conducted by a competent person. The purpose of the
courses is to acquaint potential issuers & receivers with the company internal work
permit system,
Work permit certificates will be issued internally to employees confirming them as
issuers & receivers as appropriate.
x Any person issuing or receiving a work permit must have in his possession a valid
