COLLABORATIVE PLATE INSTALLATION PROCEDURE


1. AIM

Establish the procedure related to the Installation of Collaborating Plate that applies to the “ Markham College Lower
School Remodeling ” Project.

2. SCOPE

This document must be disseminated, implemented and its compliance will be verified by all personnel involved in the
activities of the Project: “Remodeling of Markham College Lower School”.
All biosecurity measures regarding COVID-19 will be taken.

4. DEFINITIONS

✓ Accident: Any organic injury or functional disturbance caused in the workplace or during work, due to unforeseen,
fortuitous or occasional action of an external, sudden and violent force that acts suddenly on the worker's person or
due to the effort itself.

✓ Job Safety Analysis (AST): It is a risk analysis tool of a sequence of activities, applied by workers directly involved
with the stages of a job. Where the dangers are identified and their consequences present in said activity are
described.

✓ Collaborating Plate: It is a ribbed metal plate that acts as a base for the formwork, to form the collaborating slab
together with a layer of concrete. It is composed of a structural steel laminate.

✓ Shear Connector: These are Nelson Stud type bolts, which allow a permanent connection between the concrete slab
and the metal support beam. Allowing these two elements to work together. Prevents vertical separation between the
slab and the metal beam.

✓ Edge Stop: It is a structural closure accessory that serves as perimeter formwork for the concrete slab, providing a
galvanized metallic finish.

✓ Closing Stop: It is a structural closing accessory, which allows permanent closure of the slab sector where the
collaborating plate reaches the support beams.

5. WORK PROCEDURE

• Step 2: The collaborating panels will be moved from the storage


area to the final installation location, as follows:

➢ Manual: These will be hoisted using


ropes, taking care not to damage the edge. For this form of
lifting, the rope must be tied in a cross and the plate is
secured with the end of the rope. The use of leather gloves is
mandatory for the personnel in charge of lifting.

➢ Mechanical: It will be carried out using the


GROVE telescopic crane. In this case the plates will be lifted
in small packages and care must be taken not to damage the
side tabs during handling. The use of leather gloves is
mandatory for the personnel in charge of lifting.

• Step 3: Trace and Drilling of collaborative plate.

To locate the perforations, traces must be made on the


collaborating plates, taking into account the location of the
cutting connectors welded to the metal beam.

After the marking, the drilling of the collaborating plate will


begin, a process that must be carried out using a core drill
bit.

• Step 4: Placement of the Collaborating plate.


It corresponds to the definitive stage for the location of the collaborating plates on the support
beams, that is, the position in which they will remain.

The already perforated plates will be placed over the ¾” STUD Bolts already installed with
the smaller valleys on the support beam.

• Step 5: We will begin by placing the largest flange of the first plate, at the end of the beam
parallel to it, to allow the largest flanges of the subsequent sheets to fit over the smaller ones.
• The minimum support on the terminal transverse beams is 4 cm, which will remain embedded
in the slab.
• Longitudinal cuts can be made with a grinder.

• Step 6: Installing Edge Stops .

The edge stops will have a depth equal to the finished slab, they
will be placed at the ends of the collaborating plates so that at
the time of pouring, the concrete does not spill.

Note:
1.-It must be considered that, in the installation process of the
collaborating plate, edge stops will be carried out using welding
points as appropriate.

2.- Nails with primers will be placed in specific areas in case the
plate does not sit well on the beam.

3.- For points where the plate is overhanging, DVC will install
props.

7. SAFETY, OCCUPATIONAL HEALTH AND ENVIRONMENT GUIDELINE

Indicate the operational controls to consider in matters of Safety, Health and Environment.

7.1. SAFETY OPERATIONAL CONTROL

7.1.1. BEFORE THE START OF DAILY ACTIVITIES.

✓ Disseminate and Record this procedure to all personnel involved before starting the activity.

✓ Ensure that all personnel under your charge know, understand and comply with this procedure.

✓ Minimum safety equipment must be available for the start of activities. (work uniform with reflective elements,
helmet with chinstrap, safety shoes with steel toe caps, glasses, earplug.)

✓ Before starting their work, all personnel must receive the 5-minute Safety talk given by the Resident, Field
Engineer, Foreman and HSE Supervisor.

✓ The RS must carry out a tour prior to the activity to ensure route controls (lookouts, berms or safety walls, traffic
regulatory signs, etc.) as well as the positive and negative slopes, evaluating the possibility of improving these
slopes if necessary. .

✓ The Crew Leader prepares the safe work analysis (AST), with the participation of all his colleagues involved in the
task, identifying risks and establishing correction and control measures, as well as other highly relevant safety
permits.

✓ Workers must additionally receive recommendations according to the work to be performed, in order to obtain
understanding, knowledge and ensure that they have the ability to perform such tasks in a safe manner.

✓ Verification of the work area, it must not be obstructed, delimited with tape and signage according to the activity to
be carried out.

✓ The Tools will be inspected and marked according to the color code in force for the month of application as
evidence of their operability and good condition. This will be done depending on daily, weekly or monthly use.

✓ Work lifting manual loads above 25Kg per person will not be permitted; work must always be carried out with a
minimum of 02 collaborators, safeguarding ergonomics in the activities.

✓ All personal protection elements will be inspected periodically and recorded in the corresponding format,
especially in high-risk activities.

✓ It is mandatory for workers to maintain order and cleanliness in their work areas (before, during and after their
work).

✓ It is the responsibility of the Foreman, Field Supervisor and/or Group Leader to verify that the provisions of this
procedure are complied with, under the responsibility of current legislation and applicable corrective measures.

✓ HE must have the modulation of the scaffolding as a reference to the use it

✓ HE will have the MSDS sheet of the products that HE they will use in construction site.

✓ HE There will be an eyewash on site.

✓ For night work or where there is little light HE will use the glasses clear.

7.1.2. DURING THE EXECUTION OF DAILY ACTIVITIES.

7.1.2.1. GENERAL RECOMMENDATIONS

✓ The AST and other formats to be used, duly signed by those in charge, will be placed in a visible place.
✓ The transfer or passage of personnel under the shadow of falling material is prohibited.
✓ The material must be stacked in an organized manner within a safe area in such a way

that does not impede the passage of personnel and equipment.

✓ Running through the work area is prohibited.

✓ Any activity where the working conditions change will be re-analyzed by AST and the required permits will be
issued if you do not have them.

✓ Luminaires must be available in sufficient quantity for night work and in places where required.
✓ The personnel involved in the task will have all their basic and specific PPE if required.

7.1.2.2. CONSIDERATIONS FOR WORK USING HAND TOOLS:

✓ Inspect and color code all tools monthly, verifying.


✓ daily its operation, do not use tools in poor condition, defective and/or spells.
✓ Change tools that are visibly defective or in poor condition.

✓ Give each tool the use for which it was designed.


✓ The required PPE will be used to handle each tool.
✓ The handles must be free of cracks that could cause them to break.

✓ The handles must be fixed with a wedge that guarantees that the metal part will not be thrown off.
7.1.2.3. CONSIDERATIONS FOR WORK USING POWER TOOLS:

✓ Inspect and code all tools according to monthly color code, verifying their operability daily, do not use tools in poor
condition, defective and/or spelled.

✓ The power cable covers will always be in good condition.

✓ Protect electrical conductors from burns, cuts, passing vehicles, water.

✓ Avoid placing conductors on pipes or metal objects that facilitate leakage currents.
✓ Do not leave tools connected if you are going to stop using them.

✓ In humid environments or with the presence of water, do not use power tools.

✓ If they are connected to high sensitivity differential devices (30 mA), they will not require any other type of
protection. Nor if they do not have internal or external circuits that operate with a voltage greater than 50V.

✓ Before using a tool, verify that the switch is in the “Off” position.

✓ Do not use tools with cracked, broken, defective, or uninsulated handles.

✓ Do not use power tools near combustible or flammable materials.

✓ Change bits, disks, etc., with the tool stopped and unplugged.

✓ Never use discs or wheels at a speed higher than that indicated for them.

✓ The discs and grinding wheels will be free of cracks and cracks throughout their entire length.

✓ The extensions should be hung at high points, avoiding contact with the body, metal structures, or wet parts.

7.1.2.4. CONSIDERATIONS FOR WORK AT HEIGHT:

✓ Before starting any activity above 1.8 m, the PETAR for work at height and Check List of harness, ladder, lifelines,
height, etc. must be generated.

✓ Use a safety harness and lifelines with a double line and impact absorber above 1.80 m, always be hooked to an
anchor point.

✓ Mandatory use of the chinstrap on the chin.

✓ Delimit the work area where any activity is carried out at height; if necessary, lookouts will be implemented to
restrict the passage of people who are not involved in the activity.

✓ All tools, materials and/or equipment must be secured with halyards, preventing them from falling.
✓ The use of mobile phones for calls or taking photographs is prohibited.

✓ Lifelines and anchor lines must be installed and inspected by authorized personnel. They must not be used for any
purpose other than fall protection.

✓ Leaning and anchoring on the railings is strictly prohibited.

✓ Do not carry out the transfer of scaffolding with pulleys, having collaborators on the scaffolding.

✓ The anchoring elements will be capable of supporting the minimum established loads:

• Anchor Connector. - must have a resistance of 2,270 Kg. (5,000 lb.)


• Anchor point. - must resist 2,270 Kg. (5,000lb.) for each connected worker.
• Life line. - must support 2,270 Kg. (5,000lb.) for each connected worker.

✓ A piece of equipment that has arrested a fall must be immediately removed from service.

✓ It is prohibited to leave or store materials on structures, roofs or others that are located above the heads of workers.
7.1.2.5. CONSIDERATIONS FOR WORK WITH MACHINERY:

✓ Machine operators must be qualified and authorized personnel.

✓ Inspect and carry out Check List of machinery.

✓ The field manager will carry out the prior planning of the work, considering the following aspects:
Of the machinery and equipment:

▪ To place concrete, the mixer vehicles must be positioned with the help of a lookout, then place their safety
cones and blocks on the tires.
▪ When a stationary mixer is used, it must be positioned, stabilized in its working area with blocks and
marked correctly.
▪ When concrete is being placed, people should not be allowed to pass through the side of the vehicle in

✓ The permits and formats generated will be delivered to the SSOMA responsible for the Work for archiving.

✓ The workers will go to the locker rooms where they will change clothes, they will notify the person in charge or
manager of their departure from work. Afterwards they will be able to leave the work. Personnel must comply with
the work entry process, documentation with HR, current SCTR, new man safety induction, basic and specific PPE
for risk tasks.

7.1.4. OPERATIONAL HEALTH CONTROL

✓ The worker must be fit for the work to be carried out, the worker must communicate any restrictions that may
complicate his physical integrity.

✓ At the time of emptying, the worker must have his Tyvex suit, rubber boots, and face mask.

✓ Every worker must immediately inform his immediate supervisor and he

7.1.5. ENVIRONMENTAL OPERATIONAL CONTROL

✓ The teams must have the entry operation checklist and must also carry out daily reviews.

✓ Implement mixer trough washing area.

✓ All equipment must have its anti-spill kit for hydrocarbons.

✓ All hazardous waste must be segregated in the hazardous waste container and
evacuated by the contracted EPC company.

✓ The teams must comply with the execution of their scheduled maintenance for equipment and vehicles.

✓ For the continuous preventive maintenance process such as greasing, fuel supply, the equipment must have a
container system for hydrocarbon spills.
