Consent Form for Performance

My student is interested in auditioning for “Beauty and The Beast” by Alan Menken

and Tim Rice. I understand the content of the musical and that my student will be

required to attend all rehearsals and the performance unless a medical note is

provided. I consent to the context of the play with the understanding that murder,

violence, religion and romance are depicted on stage within the realm of a G to PG

rating.
