I am hereby writing to you to formalize my Claim regarding RETURN OF MONEY that I deposited in
a Deposit ATM installed in the Main Office of Ica on September 20, 2012, at approximately 8:00
p.m., NOT HAVING ISSUED THE corresponding VOUCHER , let alone MY CARD RETURNED, and
that said bank deposit does not appear on my Account Statement; The content of this complaint
that I have formulated electronically , without prejudice to the verbal complaint , is as follows:

1750 and the deposit teller at the main office of Ica in the av. Grau 135 ica I kept my

date. on 10-9-2012 and it turns out that they declared my claim inadmissible that I did
not keep any money and there is even a precedent of this bad device that they have
working that on September 10 in progress I was left with 800 soles and they returned
it to me on the 12th of the current month, for this reason, no person wants to assist the
ATMs to carry out any transaction because these ATMs usually fail in the same way,
which is why I am forced to hire a lawyer to file an appeal against this injustice and
that they return my money plus the damages and losses caused, costs and costs, plus
the sanction that INDECOPI must impose for the poor service that they have been
providing in said ATMs that one as a client is going to deposit and runs the risk of being
assisted because In the end, the cashier keeps the money and card or these blessed
