1 INSTRUCTIONS
1.1 Fill out this form in its entirety, without making any modifications, and attach the requested documents,
placing them in separate folders identified with the name of the corresponding annex (See indications in List
1 “Annexes to the Application Form). Consult the website www.onac.org.co to confirm the current version
of the application form .
1.2 Submit and file the signed application form along with the requested documentation described in List 1, in
two (2) copies on independent magnetic or electronic media (CD, DVD or USB memory).
1.3 By signing this form, the Conformity Evaluation Body (OEC hereinafter) requests a quote for the provision
of the evaluation service for initial accreditation or expansion of its scope, in accordance with the provisions
of the RAC Accreditation Service Rules. -3.0-01 .
1.4 For the review of this form and its annexes to be carried out, it is required that, in advance, the amount
equivalent to half (0.5) day of evaluation has been paid, in accordance with ONAC's current rates (see RAC-
3.0 -02 “Fees for accreditation services”) . Place the payment support in a folder identified as annex L.
1.5 The timeliness, veracity and accuracy of the information declared and included in this form will allow the
evaluation activities to be properly quoted and planned.
1.6 ONAC complies with the requirements of IAF, ILAC and IAAC for the recognition of accreditation
certificates, therefore, when completing this application the OEC must take into account that the mandatory
documents indicated in section 14 “Declaration of the legal representative” apply. of the OEC” of this form.
1.7 The email addresses registered by the OEC in this form will be the only ones officially used by ONAC for
all purposes of communication with its organization.

NOTE 1:
- In the event that the OEC changes the email addresses, it must promptly notify ONAC through formal
communication signed by the legal representative. ONAC does not assume any responsibility when the OEC
does not respond to email messages sent to the addresses indicated in this form or those notified for updating,
and that, as a consequence, the normal development of the accreditation service is affected.
- Do not leave blank spaces. If not applicable, record 'N/A'.
- If the documentation included in each of the folders identified as an annex according to List 1 is incomplete, or
the information recorded in this form presents any inconsistency, ONAC will request the OEC to complete or
adjust it, by means of an email addressed to the account registered in the form or the one notified for updating,
in order to process the accreditation request. If no response is received within ten business days, counted from
the sending of the email to the OEC, ONAC will proceed to file the OEC request and will return a copy of the
information provided by certified mail. The second copy received by ONAC will be filed as evidence of the
withdrawal of the accreditation process.

3 SCOPE OF ACCREDITATION
3.1 Scope Description: Record accurately and unambiguously in the FR-3.0-02 “Scope of Accreditation”
format. n calibration laboratory” the calibration code(s) for which the OEC requests accreditation or
extension. See List 2 “ Calibration Scope Codes ” of this form.
Place the completed form in a folder identified as ANNEX EITHER T.
3.2 Supporting documentation for the scope that is requested to be accredited.
3.2.1 For calibration methods based on standardized documents, include in a non-protected file the report
with the results of the confirmation parameters of said methods.
3.2.2 For calibration methods based on non-standardized documents, include in a non-protected file the
report with the results of the validation parameters of said methods.
It is requested that these files are not protected in order to verify the numerical information that is
registered, when applicable.
Place the confirmation report and/or method validation report in a folder identified as ANNEX O.
4.1.1 Fill out the form FR-3.3.1-03 “Monitoring the implementation of the plan for participation in
proficiency tests by the OEC” , in such a way that it covers, for the requested scope during the period
of validity of the accreditation, participation in Aptitude tests in accordance with CEA-3.0-04.
4.2.1 Attach the report(s) of proficiency test results issued by the selected provider in accordance with
Decree 1595 of 2015, and obtained in the last year, as well as evidence of the participant code. If you
have not yet received the report from the supplier, attach supporting documents evidencing
participation in proficiency testing.
4.2.2 Exceptionally, for cases in which there is no proficiency testing provider at a national or international
level, which complies with the provisions of Decree 1595 of 2015 and offers proficiency testing
program(s) that satisfy the needs and scope of accreditation of the laboratory, it may request ONAC to
evaluate the possibility of accepting only the quality assurance activities implemented, excluding
participation in proficiency tests. The request is made using form FR-3.0-18 "Partial justification for
not participating in aptitude tests"

5.2 Identify the different laboratories that exist in the different headquarters or in the same headquarters
where you carry out activities for which you request accreditation or extension before ONAC, as well
as the complete address of each of them.
5.3 Register the number of laboratories (mobile units) for which you are requesting accreditation or
extension: ( ).

5.4 Identify the different laboratories (mobile units) for which you are requesting accreditation or extension
before ONAC and the area covered by each of them. If you depend on an accredited laboratory (record
the letter A), if you depend on a non-accredited laboratory (record the letter B) and if you depend on an
organization whose main corporate purpose is not to provide calibration laboratory services (record the

8.2 Declaration that includes all activities other than those for which accreditation or extension was
requested before ONAC, carried out by the laboratory or the organization to which it belongs, stating
that said activities do not compromise the impartiality and confidentiality of the laboratory in the
development of the activity for which you requested accreditation or extension before ONAC.
8.3 List of all related organizations (1) whose activities may compromise the impartiality and confidentiality
of the calibration results for which accreditation or extension is requested before ONAC and the
measures adopted to maintain due impartiality and integrity, if applicable.
8.4 Description of the means by which the laboratory obtains resources for its operation.

The relationships could be the linking of the laboratory, by having some or all of its common owners,
common directors, contractual agreements, a same name, informal agreements or other means, by which the
related organization has some interest or has the capacity potential to influence the results of the calibrations.
9 MANAGEMENT SYSTEM
9.1 Attach the updated master list, in the format used by the laboratory management system, of all documents
established for compliance with the applicable requirements of the NTC-ISO/IEC 17025:2017 standard
that includes: code, edition and date of approval of the current version of each document. Likewise, attach
the Management System Manual, or the document that includes, references or links all the documentation,
processes, systems, records, related to compliance with the requirements of the NTC-ISO/IEC 17025
standard, the procedures ( technical and management), the manual of functions or position profiles and the
formats referenced in the list

9.2 Complete the forms FR-3.3.1-01 “Technical expert checklist – stage 1” and FR-3.3.2-05 “Leading
evaluator checklist for testing or calibration laboratories” . Record in the “Column to be completed by the
OEC” the document (Manual, procedure, format, etc.) of the OEC or activity that satisfies the requirement
of the NTC-ISO/IEC 17025:2017 standard or the specific requirements of ONAC.
9.3 Attach the following records: the report of the last internal audit that includes the activities to be
accredited or expanded and the record of the last review by management.
9.4 For each of the fixed locations and each of the magnitudes and methods that you wish to accredit or
expand, complete the following:
9.5 For each of the mobile units defined in section 5 (when applicable) and each of the calibrations that you
wish to accredit or extend, complete the following:

NOTE. In the evaluation for granting, expansion and surveillance, ONAC will require the OEC to inform
in advance the location of the mobile units.
9.6 Complete the FR-3.2.1-01/02 ANNEX J form completely before (FR-3.2-01/02-ANNEX J) “List of
regulatory documents for testing or calibration laboratories” , and attach a copy of the documents that have
been been referred to in the FR-3.0-02 format “Scope of calibration laboratory accreditation” such as a
national or international standard, regulation, own method or another referenced one.
When to preserve copyright it is necessary to provide a technical standard in physical media, indicate this
circumstance and proceed accordingly to send it to ONAC. This will be returned to the OEC once stage 2
of the evaluation is completed.

A numerical example of the estimate of the lowest uncertainty used to determine the CMC of each
magnitude included within the scope to be accredited or expanded. It is requested that this file is
not protected in order to verify the registered numerical information.
The FR-3.2-02 format – ANNEX N “Presup or this uncertainty for calibration laboratories”
completely completed.

10 LIABILITY INSURANCE
10.1 The Laboratory must establish, at its own expense, an Extracontractual Civil Liability insurance policy
issued by a company authorized by the Financial Superintendency of Colombia to operate in the
country, which is supported within the framework of the Contract of Use and Granting of the
Accreditation Certificate that they subscribe. the Conformity Evaluating Bodies and ONAC.
10.2 The coverage must be for an amount of no less than five hundred (500) legal monthly minimum wages
in force at the time of issuance or renewal. The policy must coincide with the accreditation period of the
conformity assessment body. In any case, the policy must be issued with a minimum validity period of
one year, with a note of being renewed, in such a way that it remains in force during the time of
permanence of the accreditation granted and must have attached the general clauses, the protections and
exclusions, as well as proof of premium payment.
10.3 The Laboratory must send ONAC a copy of the policy and its periodic renewal, prior to its expiration,
even if said date does not coincide with that of the surveillance evaluation.
10.4 If at the time of submitting this form the requesting organization does not have the aforementioned
Extracontractual Civil Liability insurance policy in force, it must attach a letter of commitment issued
by an insurance company authorized by the Financial Superintendency of Colombia to operate in the
country and signed. by its legal representative, declaring that in the event that ONAC grants
accreditation to the requesting organization, the company it represents undertakes to issue the insurance
policy.
10.5 In any case, the requesting organization must have the aforementioned Extracontractual Civil Liability
insurance policy in force, prior to the issuance of the accreditation certificate, that is, ONAC will not be
able to sign the use contract for granting the accreditation certificate and/or their respective annexes

until the Conformity Evaluating Body delivers to ONAC a copy of the issued policy, if accreditation is
granted.
10.6 In the event that the regulatory entities establish the constitution of professional civil liability policies as
part of the technical regulations, said policy must be presented with the application, under the
conditions of articles 2.2.1.7.8.5 and 2.2.1.7.8.6 of Decree 1595 of 2015.Place the requested

11 LAB EXPERIENCE
Complete completely the FR-3.2.1-02 form - ANNEX S “Report of results – Calibration laboratories”.
Attach a copy of the last calibration certificate issued to the client for each magnitude included in the
requested scope.

NOTE: It is mandatory that the laboratory has carried out calibrations and issued the respective calibration
certificates in each of the quantities and calibration methods for which it requests accreditation or extension of
the accredited scope, with the exception of the regulatory field or sector.
