1. PRESENTATION

The "Training on Safe Operation of Internal Combustion Forklifts" Course, given to the staff of the
company SAN FERNANDO SA , was carried out at the company's facilities on April 27, 2019, which was
led by the instructor Daniel Moreno Villajulca.
2. COURSE OBJECTIVES

The course was designed with the objectives of:

 That the company obtains competitive advantages.


 Provide participants with adequate training so that they are able to operate and inspect the
Forklift safely, reducing the risks associated with its operation.
 That the participants know the different types of Forklifts and their characteristics; the
constructive, constitutive and security elements; driving standards and compatibility with
workplaces.
 Train participants in knowledge of the applicable standard for each case.
 Awaken your interest in continually updating and perfecting your knowledge.

3. METHODOLOGY

The course was prepared for a duration of 08 hours, distributed in 04 hours for the theoretical part, 04
hours for practical training, with an approximate time of 15 minutes for the evaluation of each
participant.

For the development of the course, the standard has been taken into consideration:

 ANSI/ITSDF B56.1 “Safety Standard for Low Lift and High Lift Trucks” 2016 Edition

Likewise, the following standards have been considered:

 OSHA Standards - 29 CFR Part 1910.178 Gear Certification


 Operation Manuals, Manufacturer's Recommendations and
 Good engineering practices.

4. SUMMARY TABLE OF THE QUALIFICATION SYSTEM

Scale based on 20 points (vigesimal). Minimum passing grade 15 (represents 75%)


 Theoretical Exam (ET). Minimum passing grade 15, weight 45%
 Practical Exam (EP). Minimum passing grade 15, weight 55%
 Final Grade = (ET x 0.45) + (EP x 0.55)

Theoric exam ≥ 15 (75%)
Practical test ≥ 15 (75%)
Final note ≥ 15 (75%)
disapproved ≤14 (70%)

6. CONCLUSIONS

 The service was based on the training of “Safe Operation of Internal Combustion
Forklifts” , specifically of the Brand: TOYOYA, Model: 328FG30, Capacity: 3 t.
 Once the training was completed, the applicants who managed to achieve the necessary
score that qualifies them as Forklift Operators demonstrated that they have the level of
knowledge and skills to carry out operations safely.
 The proposed course and training objectives (theoretical and practical) were met in the
content and deadline established for the group of scheduled operators.
 The attending staff showed interest by participating in the analysis of the points
discussed; it is worth highlighting the effort of the entire group in showing their skill in the
operation and handling of the forklift.
 The staff recognized the minimum requirements to carry out operations with the
equipment, which lead to avoiding incidents and/or accidents.

7. RECOMMENDATIONS

 Before fully enabling the personnel, carry out verification and disqualify the
personnel who are found to be unfit from the medical examination carried out where they show
mental, physical problems, among others.
 Keep your Forklift Operators in constant training, through a Training program; It is
considered a regulated and monitored activity in all industrial activities.

 Keep the operators who reached the minimum passing score in constant practice
so that they can solidify their skills and respond to their needs.
 Always consider frequent, periodic, and predictive maintenance of the Forklift, which
will help minimize risks and strengthen its operations to perform handling maneuvers in safe
conditions.
 Take into account the suggestions of qualified operators regarding safety related to
good practices for material movement and lifting, maintenance and frequent inspections.
 Mandatory enforcement of the pre-use inspection (Check – List) of the Forklift
