SPES REMINDERS:

➢ Photocopy of birth certificate provided must be readable. Local and PSA/NSO copy
are both accepted.
➢ Grades from Kiosk, system generated grades or class cards can be accepted upon
submission of documentary requirements for screening and validation.
➢ Original Certificate of Grades 1ST SEMESTER SY 2022-2023 with original sign and
stamp from school registrar shall be submitted before signing of contract schedule.
➢ In case of student can’t provide his/her 1st Semester grades for SY 2022-2023, latest
grades should be provided with an attachment of Certificate of Out of School Youth
issued by MSWDO or Barangay Official.
➢ Date Issuance of BIR’s Certificate of Tax Exemption with an attachment of
Certificate of Indigence from respective barangays should be dated on before the
date of Contract Signing. SHOULD BE NAME TO BOTH PARENTS -if living
together, name to mother or father If solo parent or separated OR name to
GUARDIAN if not living with parents or both parents were deceased.
➢ If parent/ parent’s are employed to Government/ Private company ITR BIR FORM
2316 should be provided instead of Certificate of Tax Exemption.
➢ If living with GUARDIAN- Affidavit of Guardianship will be required, issued by
Barangay OR Office of the Municipal Mayor.
➢ Photocopy of Student’s ID with specimen signature.(Match signature is a MUST)
➢ Photocopy of Parent OR guardian’s ID with specimen signature. (Match signature is
a MUST)
