AFFIDAVIT

I, Smt. Shakuntala Ganesh Thakur, Age 40 years, occupa -

tion- household, residing at –Village Sanpada, Taluka and Dis -
trict Thane, Thane Belapur Road, Navi Mumbai, do hereby
solemnly affirm as under: -

1. I say that I am residing on the above address with my family. I

say that I have registered my demand for accommodation with
CIDCO under DRS-87 Scheme and have paid Rs.1000/- vide Re -
ceipt No. 45955 in the year 1987.

2. I say that the said Receipt No. 45955 is lost from me in the
transaction and hence this affidavit is made as required
to produce before the component authority of CIDCO for the
proof and purpose thereof.

Whatever stated here in above is true and correct to the

best of my knowledge, belief and information.
