Preparation
A typical formulary must have the following composition;
1. Title page
2. Names & titles of the members of the PTC
3. Table of contents
4. Information on hospital policies & procedures concerning drugs
The Pharmacy and Therapeutics Committee of hospital.
Objective and operation of the formulary system.
Hospital regulations and procedures for prescribing and dispensing drugs.
Hospital pharmacy services and procedures.
How to use the formulary.
5. Products accepted for use at hospital
Items added and deleted from previous edition.
Generic, brand name, cross reference list.
Pharmacologic/therapeutic index with relative cost codes.
Descriptions of formulary drug products by pharmacologic therapeutic class.
6. Appendix
Central service equipment and supply list.
Guidelines for calculating paediatric doses.

Revision
The members alone are not competent to evaluate each therapeutic agent.
Any preparations whose formula is not disclosed cannot be entered in the
formulary.
PTC will decide the revision of formulary
Meeting is held by PTC for the revision
Annual revision is done and is must because of alterations in drug
products, removal of some drugs from market, new drugs and changes in
hospital policies
Drug details are added at the end of formulary by attaching the sheets at
the back or last of formulary
The addition or deletion of drug is only done after reporting to medical
staff

Addition And Deletion
The following criteria should be considered while adding into or removing
drugs from the formulary:
1) The medical staff should have approved the drug to be added in the
formulary.
2) The drug should be recognised by the Pharmacopoe ias and Formularies
approved under Drugs and Cosmetic Act and Rules.
3) The drug manufacturer should hold a license issued under the Drugs and
Cosmetic Rules. Also he should not have been punished for any serious
offence under any law of Drugs and Medicines.
4) The drug or preparation of secret composition should not be added in
the formulary.
5) The drug or preparation containing multiple ingredients should not be
added if the same therapeutic effect can be obtained by using a
