Note 1. The fields that have (*) must be filled out on a computer before being printed.

Note 2. In extraordinary cases, a maximum of 50% of the total number of (tourists/excursionists)

entered manually will be considered (as long as they leave from the same starting point),
considering them subject to the same terms and conditions of service, as well as the same
benefits that the service contract entails

Note 3. In the event that the contracting party decides to change the original destination for
which the contract was made, this must be filled out on the back where it indicates "NEW
DESTINATION" which must be filled out with a pen and signature of the contracting party.

Note 4. In the event of an unexpected change of driver, tour guide and/or tour leader, this
document must be accompanied by an email (printed and attached to the route sheet) indicating
the individual.

Note 5. In the event that the legal representative of the tourist land transportation operator
cannot sign the route sheet, another person must be delegated to a notary public.

Note 6. The veracity of all the information contained in this form is the absolute responsibility of
the Legal Representative and the drivers of the vehicle.

Note 7. The National Agency for Land Transportation, Traffic and Road Safety and any road
control entity reserve the right to indicate the pertinent legal action, in the event that any
adulteration is detected in the information on the form.

Note 8. If the contracting party is a tourist services agency, the tour and optional fields (if
available) must fill out the tour leader's information.

Note 9. This form must be presented to the control entities in the territory.

Note 10. The number of passengers must not exceed the number of seats available on the vehicle
registration.
