TO WHOM IT MAY CONCERN:

THIS IS TO CERTIFY that based on record dated April 19, 2024, the following
data exists and is extracted to read:

“REPORT re LOST WALLET” On April 19, 2024 at about 1:07 PM, Mr.
REAGAN A. ADDAWE, of legal age and a resident of Bunot, Paracelis, Mountain
Province personally appeared before this police station and requested to put into
records that his wallet containing several Identification Cards (IDs) all under his
name most particularly his PROFESSIONAL DRIVER’S LICENSE card issued by the
Land Transportation Office is missing. Accordingly, sometime on the morning of
March 27, 2024, he was at Roxas, Isabela when he noticed that said wallet is
missing from his possession. Above-named reporting person further stated that
despite diligent effort to locate the same, it proved futile and is now considered lost
and can no longer be found. Further, the said Professional Driver’s License was lost
in the manner stated above and was not confiscated by the issuing agency for any
violation of its rules and regulations.

This certification is issued upon request of the above-named person this 19 th

day of April 2024 for replacement of the said license card.
