1 Regulatory basis
• Articles 5 and 6 section V and article 82 sections III, VI, XV and XVI of the Internal
Regulations of the Mexican Social Security Institute published in the Official Gazette of the
Federation on September 18, 2006 and its reforms.
• Paragraphs 13, 14 and 15 of section 8.1.5.1.1 of the Organization Manual of the Medical
Benefits Directorate, code 2000-002-001 validated and registered on February 8, 2018.
2 Aim
Carry out the planning and definition of personnel templates, in medical units at the three
levels of care that are new works, replacement, expansion, remodeling, by donation of real
estate, expansion associated with the incorporation or replacement of medical equipment
based on the current institutional guidelines and in harmony with the requirements of the
General Health Law and applicable official standards , in order to provide the necessary
personnel to operate the corresponding health units.

4 Definitions
4.1 work action : Corresponds to new works, replacement, expansion, remodeling and
donation of buildings (Family Medicine Units, Ambulatory Care Medical Units, second
level Hospital Medical Units, High Specialty Medical Units, Detection and Diagnosis Unit
of Breast Cancer, Ambulatory Metabolic Control Unit and other types of units according
to the new authorized models), by Central Level or by the Deconcentrated Administrative
Operation Body.
4.6 CEPI : Physical Investment Project Evaluation Card, which can be Medical (M) and
Public Works (OP).
4.12 Nursing staff distribution table by service, shifts and categories: Administrative
technical document, which systematically organizes the nursing and orderly staff, based on
institutional indicators, determined by physical capacity, productivity and operational level.
4.19 minimum staff: Management group of a medical unit, which is hired to begin
administrative activities in new units or by donation of property.
4.20 Authorized workforce: Number of human resources by category, shift and type of
position, initially determined for the operation of the medical units, according to the services to
be provided; information that may vary derived from the application of medical programs,
strengthening of services or final analysis of the workforce.

5 Policies
5.1 General
5.1.1. The entry into force of this document will update and render void the “Procedure for the
planning and definition of personnel templates for medical units subject to construction action”,
with code 2900-003-002, published on October 16, 2013 .
5.1.3. Non-compliance by the public servants involved in this document will be cause for the
responsibilities that result in accordance with the General Law of Administrative
Responsibilities and other applicable provisions in this regard.
5.1.4. The language used in this document, in the annexes and formats, does not seek to
generate any distinction or mark differences between men and women, so the references or
allusions in the wording made towards one gender represent both sexes, except in those
cases in which, due to the nature of the attention, precision of some particular kind is
necessary.

of the Mexican Social Security Institute, code 2200-003-002, validated and registered on July
11, 2019.
5.2.1. It will be the responsibility of the regulatory Coordinations of the different Directorates
whose work process is immersed in the care of the beneficiary in a medical unit, to inform the
Medical Infrastructure Planning Coordination of the inclusions or modifications of the human
resources provision indicators of the categories. under their charge, which must be based on
institutional regulations, as well as on historical and expected productivity based on their
functions.
5.2.2. It will be up to the Personnel Unit, through the Coordination of Budget and
Management of Expenditure on Personal Services, to manage the positions considered in the
preliminary draft of the Works Program.
5.2.3. The growth of places in medical units in operation, which is not associated with work,
will be defined and processed in a timely manner before the Human Resources Management
Coordination by the corresponding regulations of each level of care.
5.2.4. The Coordination of Conservation and General Services is responsible for determining
the staff of Conservation and Cleaning and Hygiene personnel.
5.2.5. The “Indicators of human resource provision for medical units”, code 2900-B23-002
(Annex 2), may be used as a reference for medical units in operation, it is important to note
that the installed capacity must be considered, without representing that The application of the
indicators generates commitments to carry out works in the units or increase places without the
corresponding Regulations requesting the necessary budget in accordance with the
regulations; The authorized human resources budget must also be considered.
5.2.6. It will monitor, verify and evaluate compliance with the policies and activities of this
procedure, to obtain better results in the provision of human resources.
5.2.7. It will prepare, no later than April of each year, the preliminary draft of the requirement
for places for the following fiscal year, so that after analysis by the Coordination of Human
Resources Management, the Coordination of Budget and Management of Expenditure on
Personal Services, it determines the cost and this can be included in the preliminary draft
budget.
5.2.8. It will deliver the requirement for places at the highest level of detail possible, including
the Deconcentrated Administrative Operation Body, the type of unit, the number of places by
category, the percentage of progress of the work and the probable month of inclusion.
5.2.9. In the preliminary project for the requirement of places for the following fiscal year, the
projection of work action places by equipment will be included, due to the variation in the
acquisition of equipment at the national level, the average of the places may be considered for
the calculation. granted in the previous three years.
5.2.10. It will update the “Guidelines for the allocation of physical resources and staff, based
on the medical program of the IMSS medical units”, code 2900-B13-002 (Annex 1), in
accordance with the agreed modifications, in coordination with the CAISPN, CAISN, CUMAE,
CGRH or the CCSG.
5.2.11. It will coordinate with the different regulations of the Directorate of Medical Benefits
and the Regulatory Directorates, whose categories are immersed in care in medical units, the
updating of the “Indicators of human resources provision for medical units”, code 2900-B23 -
002 (Annex 2).
5.2.12. It will coordinate the necessary activities with the CGRH, the CCSG and with the
Deconcentrated Administrative Operation Bodies involved, to determine or update the
authorized templates of the medical units with work action and work action with equipment,
considering the models of the medical units and care models, as appropriate, as well as the
incorporation of new technology, which requires staffing.
5.2.13. It will coordinate the necessary activities with the CGRH, the CCSG and with the

5.2.14. As an exception, the CPIM may authorize the review of the staff when the work
presents progress of less than 80%, when there are several works in the Deconcentrated
Administrative Operation Body and the time and resources invested by the Institute can be
optimized. or there are circumstances that make this prior analysis necessary.
5.2.15. It will inform the Human Resources Management Coordination of the start date of
operation of new works, replacement, expansion, remodeling and donation of properties of
Family Medicine Units, Ambulatory Care Medical Units, second level Hospital Medical Units,
High Specialty Medical, Breast Cancer Detection and Diagnosis Unit and Ambulatory
Metabolic Control Unit, in accordance with the information provided by the CII and/or the
OOAD, regarding the progress of the work, as well as the delivery of the equipment and the
corresponding furniture.
5.2.16. It will validate that prior to receiving the equipment in the new medical unit, the
resources of the minimum staff are incorporated by the Human Resources Management
Coordination and the Head of Personnel Development Services of the OOAD, and will cover
them.
5.2.17. It will validate and coordinate that once the equipment is received in the unit and the
start date of operation is defined, the authorized workforce is incorporated.
5.2.18. Manage the updating of the Information Units and Cost Centers before the Heads of
Finance Services in the OOAD and in the High Specialty Medical Units, the Heads of the
Personnel Department and Contractual Relations before the Head of the Finance Department,
once that have the determination of the staff of the new unit or the unit in operation, in
accordance with the “Procedure for the administration of the keys of the responsibility units in
the PREI Millenium system”, code 6B11-003-017.
5.2.19. It will inform the Human Resources Management Coordination, the Information Units
and the Cost Centers of the new or operating unit, for the creation of departmental codes in
accordance with the “Procedure for the incorporation and updating of the catalog of
departmental codes” , code 1A12-003-026.
5.2.20. It will send to the Medical Infrastructure Planning Coordination, the Proposals for the
Update to the Catalog of Places in accordance with the authorized template determined by the
medical unit with work action, in accordance with the “Procedure for the processing of
proposals for the update to the catalog of positions”, code 1A12-003-030, once the
departmental codes are included in the “Comprehensive Personnel Administration System”
(SIAP).

The assignment of human resources or staff to medical units subject to construction action or

For each type of work action in the Medical Units, prior information and activities are required
by the Deconcentrated Administrative Operation Bodies or High Specialty Medical Units,
which will allow an adequate assignment of personnel, considering the following:

of Real Estate Infrastructure, through which they report the physical-financial progress

DONATION PROPERTIES
The legal documents must have been resolved prior to adapting the physical structure
necessary to provide medical care; Therefore, when the time comes to work on the allocation
of human resources, the requirements to start activities will be the same as any property
subject to construction action.
In any of the modalities, once the aforementioned information is available, it will be the basis
for integrating the “Physical Resources” file, which will contain the services that the medical
unit will provide, indicating whether they exist in the plan (in hospitals They can be located by
floor according to the architectural plans), how many areas, census beds or offices, the shifts
in which the medical unit will be operating and comments that provide additional information
about the service.

The calculation of resources for the category of doctors for units in the process of building

1. The total number of activities carried out by doctors of each specialty in the period
analyzed is recorded and multiplied by the average times assigned for each of them,
outpatient consultation, hospitalization, surgery, tocosurgery, interconsultations, etc.
• First time consultation: At second and third level of care: for new units of 0.5 hours and for
units in function of 0.33 hours.
• Subsequent consultation: 0.25 hours.
• Hospital admission: 0.5 hours at both levels.
• Hospital discharge: 0.5 hours at both levels.

• Surgical intervention: the duration of surgery in the HMUs in operation on average is 1.5
hours per surgery, in the UMAE it is 2.5 hours.
• Resuscitation of the newborn 0.5 hours at both levels.
• For the 33 procedures, reported to the Health Information Division (DIS) currently captured
in official records, such as taking biopsies, lumbar puncture, etc.; The average medical
time assigned is 0.5 hours for both levels.
For the calculation of Gynecological obstetrician doctors, the following will also be considered:
• Tocosurgery Admission Assessment 0.33 hours.
• Ultrasound in Tocosurgery 0.25 hours.
• Tococardiographic recording in Tocosurgery 0.5 hours.
• Admission to Tocosurgery 0.5 hours.
• Tococardiographic recording 0.5 hours.
• Labor monitoring 1.25 hours (includes an average of 5 assessments lasting 10 minutes
each).
• Delivery (in expulsion area or friendly delivery cubicle): 1.0 hour on both levels.
• Surgical procedure 1.5 hours.
• Surveillance of the postpartum period in the recovery area in Tocosurgery (1
assessment every 15 minutes in the first hour, one assessment every 30 minutes in the
second hour and an assessment of discharge from the service in the third hour and
considering discharge from the area) 10 min .
The procedures must be recorded in source formats and in the clinical record.
2. The sum of hours needed is divided by 250, which corresponds to the total number of
working days for a doctor in a year. The result is equivalent to the number of medical hours
required per day.
3. The resulting number of hours is divided by 6.0 hours, which is the effective daily working
hours of each doctor, and the total number of doctors needed is obtained.
Example: In the specialty of General Surgery at a UMH, with the following activities for one
year:
3,341 first-time consultations and 5,274 subsequent consultations, 831 hospital discharges
with 11,815 patient days, 5,311 surgeries and 2,190 interconsultations were carried out.
These specialists must be distributed between shifts; taking into account the availability of the
medical program and the programmable activities per shift when making said distribution.
Considering that in the night shifts and on days of accumulated work hours, there are usually
no scheduled activities such as consultation, the provision of medical personnel for them, in
the four basic services, is carried out with the criterion of obtaining coverage to attend to the
services that they require. are authorized to operate, as well as the emergencies inherent to
the service and those generated in other areas.
To carry out the productivity study of this staff, specific data on what was done in said shifts is
required, which will serve to evaluate possible variations in the number of doctors.
Emergencies
▪ At the second and third level of care, the calculation of doctors in the Emergency Service
has special characteristics, since this personnel must be estimated so that the services
are in operation 24 hours a day, 365 days a year.
▪ Triage is the classification of the level of severity, according to the current algorithm, for all
patients who request care in this service; which is done in a maximum time of 5 minutes;
This is carried out by the General Physician category at UMH, one doctor will be assigned
per shift to guarantee the classification of the level of severity.
▪ Resuscitation Room, area in which patients classified in red and orange (5% of demand)
are cared for, considering 1 hour for their care.
▪ The proportions in the percentage of real emergencies (red, orange and yellow) vs. felt
urges (green and blue) in this classification may be modified based on documented
history.
▪ In the Office, patients classified as yellow (35%) and green or blue (60%) will be treated:
• The time considered for the care of patients classified as yellow is 30 minutes (0.5
hour) and for those classified as green or blue, 15 minutes (0.25 hour).
• In the short-stay and patient preparation area, 15% of adult patients will be treated,
where the reason for admission will be resolved within a period of up to 6 hours, with
time considered for the care of these patients of 60 minutes; as well as the preparation
of all the patients who will be admitted to the adult observation area; except for those
coming from the Resuscitation Room who will enter this area directly.
• For the adult observation and pediatric observation areas (21% and 4% respectively of
the demand), for the continuity of patient care, with time considered for these patients
of 90 minutes (1.5 hours) for each discharge;

Example: Hospital unit whose Emergency Department had a request for 73,000 visits in the
last year.
A: 73,000 X 0.05 (5% with urgency classified as red or orange) = 3,650 patients.
B: 3,650 X 1 hour = 3,650 medical hours.
C: 73,000 X 0.35 (35% urgency classified as yellow) = 25,550 patients.
D: 25,550 X 0.5 hour = 12,775 medical hours.
E: 73,000 X 0.6 (60% emergencies classified as green and blue) = 43,800 patients.
F: 43,800 X 0.25 hour = 10,950 medical hours.
G: 73,000 X 0.15 (15% short-stay patients) = 10,950 patients.
H: 10,950 X 1.0 = 10,950 medical hours.
I: 73,000 X 0.21 (21% adult observation admissions) = 15,330 patients.
J: 15,330 X 1.5 hours = 22,995 medical hours.
K: 73,000 X 0.04 (4% admissions to pediatric observation) =2,920 patients.
L. 2,920 X 1.5 hours = 4,380 medical hours.
M: B + D + F + H + J + L= 65,700 medical hours.
N: 65,700 / 250 working days per doctor per year = 262.8 days.
Ñ: 262.8 / 6.0 hours of work for each doctor = 43.8 doctors.
The distribution per shift of the 44 positions (does not include the General Physician positions
assigned to the Triage area), one doctor will be assigned per shift in the five shifts, ensuring
continuous care, taking into account the existence of the medical program in the unit. .

Admission Tocosurgery
Birth care in the labor area or in individual care rooms, for units that already have the Friendly
Birth labor concept, are assigned the same time. The distribution of doctors will be done
equally to provide coverage in the five shifts of the seven days a week in tocosurgery and the
services that can be scheduled per shift.
Anesthesiology
It is carried out considering the following parameters:
• A doctor by shift per authorized operating room.
• A doctor by shift for every two authorized procedure rooms.
• A doctor by shift for the labor and expulsion areas.
• A doctor by shift for the recovery area for every 10 stretchers of recovery.
• A doctor by shift authorized for Preoperative Assessment Consultation.
• One doctor per shift authorized to support studies such as: Magnetic Resonance,
Endoscopy, Hemodynamics, etc.
In the event that the demand for each of the activities listed above does not justify the
provision of a doctor for your care, a schedule of rotation of activities must be developed to
comply with the resolution of care. Personnel will be provided if the physical area and
equipment are available.
Example: Hospital Medical Unit with 144 beds that has 2 operating rooms, one more room
in Tocosurgery and two expulsion rooms. The operating rooms operate 2 in the morning shift,
2 in the afternoon; one for night shift and one for cumulative shift; an Endoscopy room that
schedules studies from Monday to Friday in the evening shift and a Magnetic Resonance with
a schedule for 20 patients, in the shift that requires an anesthetic procedure.

Specific considerations
Transplants
The physical personnel resource for the Transplant Area has been established by the
Coordination of Donation and Transplantation of Organs, Tissues and Cells (CDTOTC),
responsible for this medical care process for the Second and Third Level of Care Medical
Units. Some concepts that the Coordination considers relevant to the functioning of the areas
are included:
Corneal transplant (applies at UMAE and UMH). In the UMAE, the doctors report to the Head
of the Transplant Division (if it exists) or the Head of the Clinical Department of
Ophthalmology. In the UMH, they will report to the Head of the Clinical Coordination of
Surgery.
Hematopoietic progenitor cell (HPC) transplant: Applies only in UMAE; Doctors report to the
Head of the Transplant Division (if applicable) or the Head of the Clinical Department of
Hematology. If they do not have transplant activity, which is considered their priority, they
must support with activities assigned to them by the Immediate Manager.

The basic cell is applied when achieving a minimum of 20 transplants per year when at least
50 transplants per year are achieved consistently over 2 years, the cell can be duplicated up
to a maximum of two cells. One Intensive Care Specialist Nurse (preferably with training) is
required per patient on each shift; If they do not have transplant activity, they must support
with activities in one of the Intensive Care Units in the unit.
Intrathoracic organ transplant: Applies only in UMAE; Heart and/or lung transplant is
considered in this category; The cell is applied for each active program, regardless of
productivity. It is considered an active program when it performs a minimum of three
transplants per year for at least two years. If they do not have transplant activity, which is
considered their priority, doctors must support the activities assigned to them by the
Immediate Superior Chief according to the area in which they are assigned.
The application of the “Indicators of human resource provision for medical units”, code 2900-
B23-002 (Annex 2) of this document, will be in charge of the regulatory areas of the three
levels of medical care, which will establish the mechanism review with the units in charge for

The regulatory areas are responsible for managing the identified personnel needs and their
categories before the Human Resources Management Coordination, as well as requesting or

New Programs
After the constitution of new programs or modifications of those currently in operation, the

Coordination delivers to the CTIM the list of medical equipment in projection for annual

be provided with the different categories that are related to their efficient use and that allow
meeting the needs of the unit and the units that require support from the unit.
Once the following year's allocation has been requested from the CGRH and the medical units
