EXTRACT FROM POLICE BLOTTER REPORT

TO WHOM IT MAY CONCERN:

THIS IS TO CERTIFY that based on Police record dated March 23, 2024; the
following data exist and is extracted to read:

“REPORT ON LOST PASSPORT” On March 23, 2024 at 10:00 AM, Ms.

Gleta Sogan y Tangbawan, 31 years old (DOB May 11, 1991), housekeeping and
resident of Santa Isabel, Natonin, Mountain Province appeared before this MPS and
reported that her passport got loss on February 24, 2024 undisclosed time.

Interview with above named person revealed that, she kept her passport in
her bag as she traveled from Paracelis, Mountain Province to Manila when she
accidentally left it in the bus. Upon noticed that that her bag was no longer in her
possession, she returned back to the said bus for possible recovery however, it
resulted futile. Further interview revealed that the owner of the subject item tried to
inquire to the driver and several others but all claimed to have not have not seen it.
Hence, the above named person believes to have been mistakenly taken by some
passenger in the bus considering her diligent effort to search for it. Now therefore,
the item is considered lost and beyond recovery.

This certification is issued upon request of the above-named person this 24th
day of March 2024 for issuance/replacement of the same.
