SUBJECT: Police Report

TO WHOM IT MAY CONCERN:

THIS IS TO CERTIFIY that base on record dated March 25, 2024, the following data
exists and is extracted to read:

“Alleged Grave Oral Defamation and CYBER Libel” on March 25, 2024 at about
10:24 AM, complainant Mrs. Angeline Malindao y Tamalay, 37 years old (DOB November 6,
1986) married, housewife and resident of San Rafael Bananao, Paracelis, Mountain Province
personally came before this MPS and reported that at around 4:00 PM of March 24, 2024 she
was at the Barangay Hall of Bananao, Paracelis attending barangay assembly held thereat.
That during the open forum part of the activity she manifested her intention to talked to inquire.
After she was recognized to inquire, she then asked/inquired address to the Punong Barangay
Hon John F Bautista regarding TUPAD Clearance asking why do they need to collect an
amount of one hundred thirty pesos (Php 130.00) while in fact DOLE as accordingly doesn’t
require such fee. Immediately thereafter, Hon Bautista became hysterical, uttered unsavory/
insulting words to quote “dambel, aginlalaing, ningning, tanga” in front and within the presence
of barangay officials, community residents, invited PNP personnel, Hon Vice Mayor and some
invited teachers. This cause much shame and insult that affected the reputation of the above
named complainant. Mrs. Malindao further reported that Hon. Bautista continued his act to
caused dishonor the reputation of the complainant by posting on the social media “MYDAY”
using his Facebook Account. Screenshots have been saved by the complainant to form part of
her complaint.

This certification is issued upon request of the above named complainant this ___ day
of April, 2024 at Paracelis, Mountain Province for whatever legal intent it may serve her best.

 MEMORANDUM OF AGREEMENT

Know all men by these Present:

An agreement entered into by HON. THOMAS G. SIMANGON, of legal age, Barangay

Chairman of Bacarri, Paracelis, Mountain Province and a resident of said place named herein
as the First Party Complainant and MS. AIZA G. ALIPIT, of legal age with postal address of
same place above herein as the Second Party Defendant in connection to a complaint of Hon.
Simangon for alleged MALVERSATION OF PUBLIC FUNDS OR PROPERTY that transpired
sometime in the year Nov. 2022 to Nov. 2023 at Bacarri, Paracelis, Mountain Province.

Witnessed whereas, the second party was previously appointed as Barangay

Treasurer with unaccounted/ fund released Barangay fund in the total amount of more or less
Two Million (2,000,000.00) Pesos.

Whereas, the incumbent Barangay Officials demand the payment of the aforesaid
unliquidated amount.

Whereas, In view of the foregoing, the parties had a dialogue where it was stipulated
and agreed, among others, that;

A. The first Party promise to pay/settle the said amount On or Before May 30, 2024;

B. To guaranty the payment/settlement thereof, the second party offered as collateral

Real Properties hereunder identified as follows;

1. Declaration of Real Property of Mr. Alejandro Gumaad with Tax Declaration

No. 2348 Located at Tolang, Bacarri, Paracelis Mountain Province.

2. Declaration of Real Property of Mr. Alip Gosalip with Tax Declaration No.
3557 Located at Amocao, Bunot, Paracelis Mountain Province.

3. Declaration of Real Property of Mr. Simon Gumaad with Tax Declaration No.
1265 Located at Baay, Bacarri, Paracelis Mountain Province.

4. Declaration of Real Property of Mr. Rosita Gumaad with Tax Declaration No.
3434 Located at Daowan,Baay, Bacarri, Paracelis Mountain Province.

5. Declaration of Real Property of Mr. Rosita Goliquey with Tax Declaration No.
7457 Located at Mapipit, Bacarri, Paracelis Mountain Province.

6. Declaration of Real Property of Mr. Fermin Copio with Tax Declaration No.
5295 Located at Mapipit, Bacarri, Paracelis Mountain Province.

C. Failure on the part of second party to settle the said monetary obligation, the first
party may file or cause the filing of the necessary complaint/charges against the
second party.
