UNDERTAKING

I, , of Catolohan, Dalaguete,
Cebu of legal age, Filipino, hereby undertake to implement the requested
Project being funded thru the financial assistance extended to our Barangay
by the Cebu Provincial Government within six (6) months from receipt of said
financial assistance, with respect of said to infra projects, or within thirty (30)
days relative to non – infra projects, like socio – cultural activities, purchase
of goods and the like, as the case may be.

It is understood that resolution of Sangguniang Barangay shall be

Passed whenever the requested financial assistance would be used for other
purpose subject. However, to the approval by the appropriate officials of the
Province of Cebu, moreover, any unexpected balance of financial assistance
received, will be returned to the Province of Cebu through the Provincial
Treasurer except when authorized to utilize the same.

Dalaguete, Cebu, of legal age, Filipino, hereby undertake to liquidate the
previous financial assistance received by our Barangay from the Cebu
Provincial Government within six (6) months from today, with respect to infra
projects, or within thirty (30) days relative to non- infra projects like socio-
cultural activities, purchase of goods and the likes, from date of activity or
date of purchase, as the case may be, duly verified by COA unit auditor.
