Checklist: Urinary Catheter Male & Female

1 Wash hands, introduce self and explain the procedure to the patient
Check the patient’s chart, doctor`s order & gather supplies. ►Urinary bag with
outside bag to hand it ►Urinary catheter sterile (14-16fr for adult) ►Sterile
2 dsg kit containing: drapes, tray, cotton, forceps
►Lubricant ►Iodine ►10cc syringe with NS or sterile water ►light
►Trash can
Have the patient wash genital area with warm water and diluted soap. (can say

verbally)
4 Provide privacy: DO NOT uncover patient until ready to place sterile drape!!
5 Fill syringe with 10cc of NS/sterile water & prepare tape.
Cut just the top of the sterile package for urinary catheter & connect urine bag to

urinary catheter.
7 Test balloon and remove syringe.
8 Position the patient appropriately and place sterile drape under the patient.
9 Open the sterile container: maintaining sterile technique.
10 Pour iodine on cotton balls; keep at least on without iodine. Pour form side
11 Put lubricant on tray.
12 Put on sterile gloves (quickly and smoothly; without contaminating)
13 Move container to the patient if needed.
M ► Using non-dominate hand hold penis at 90 degree angle.
F ► Using non-dominate hand separate the labia.

( DO NOT remove the non-dominate hand from position until after the catheter
is inserted and balloon inflated)
M ► clean male: in circular motion from urethral meatus down.
F ► clean female: in downward motion on left, then on right, and lastly over the
15 urethral meatus.
(Note: may dry meatus with one sterile clean cotton ball at the end of cleaning to
prevent irritation).
Keeping non-dominate hand in place, take urinary catheter with dominate hand,

touching only the sterile part and coil in hand.
Lubricate the catheter (F 3-5 or M 15-18) cm and insert catheter.

Ask the patient to take a deep breath and breathe slowly through nose. Ask the
patient to bear down gently as if to void. (after urinary catheter in urethra, of

course)
M ► pull penis upward slightly while inserting.
Insert catheter gently about (M►15 –18 or F►5-10) cm. when you see urine begin
19 to flow, insert the catheter another 3 1 /2 cm, and then inflate the balloon. Look at
the patient face while inflating balloon to make sure the patient does not have pain.
20 Pull back gently on the catheter until it is in place.
Tape to the patient `s (M ►abdomen or F► thigh). Hang urine bag from bed.

Never allow the bag to touch the floor!

Remove catheter: put on clean gloves. Empty the urine bag but leave connected
to the catheter.
23 Put towel under patient, no need to be sterile. Explain procedure to the patient.
24 Deflate balloon 2 times. Have the take a deep breath and gently remove.
Remove gloves and put catheter in gloves. Give tissue (Kleenex) to the patient to

the area.
Question related to urinary catheter in general (male and female ): for example
1) In male patient, if resistance at prostatic sphincter, continue to apply gentle
pressure with catheter and then ask patient to attempt to void. Never force
2) If no urine coming:
Female: may be in vagina. If in vagina, leave it and get a new sterile catheter to
+1 place in urethra.
Male: insert the catheter all the way and then inflate the balloon.
3) If blood is seen coming out of urethra meatus. Tell doctor and do not insert.
Doctor will insert after making sure the urethra in intact.
4) What should you document related to the urinary catheter.
