Second: General Work Permit: ‫ ﺗﺼﺮﻳﺢ ﻋﻤﻞ ﻋﺎم‬:‫ﺛﺎﻧﻴﺎ‬

1. The company/ contractor shall begin execution upon obtaining the work permit and it shall post a copy
thereof in a prominent place at the workplace; and the company/ contractor shall renew the work permit
sufficiently enough priory to its expiry.
1. The company/ contractor shall coordinate with the facility management at the time of work in the location,
so not to affect the use of the location or the facility.
1. The telephone number of the company's officer/ contractor, responsible for executing the works shall be
available to contact, when necessary, particularly in case of emergency.
1. All employees shall bear residency permits as per the State's systems and regulations.
1. The civil ID card of the engineer or the officer responsible for the work site and approved with the
company (contractor) shall be submitted, so that it shall be the sole person in charge of the actions and
violations of its employees (subcontractors).
1. Entry, exit and admission of goods and equipment with all its types shall be strictly prohibited except
through the gates designated by the facility management.
1. The civil ID cards of the employees of the company (contractor) shall be handed over to the security room
at the gates designated by the facility's management.
1. All vehicles shall fulfil all applicable General Department of Traffic laws and regulations.
1. It's prohibited to employ employees not registered on the company's (contractor) sponsorship; and in case
of subcontracting, such conditions shall be applied to the subcontractor.
1. It's prohibited to keep or store the flammable materials inside the workplace or the public areas of the
facility.

.‫ﻟﻠﻌﻴﺎن وﺗﻠﺘﺰم اﻟﺸﺮﻛﺔ )اﻟﻤﻘﺎول( ﺑﺘﺠﺪﻳﺪ اﻟﺘﺼﺮﻳﺢ ﻗﺒﻞ اﻧﺘﻬﺎﺋﻪ ﺑﻤﺪة ﻛﺎﻓﻴﺔ‬
.‫ ﺗﻠﺘﺰم اﻟﺸﺮﻛﺔ )اﻟﻤﻘﺎول( ﺑﺎﻟﺘﻨﺴﻴﻖ ﻣﻊ اﻟﺠﻬﺔ إدارة اﻟﻤﺮﻓﻖ ﺑﺄوﻗﺎت اﻟﻌﻤﻞ ﺑﺎﻟﻤﻮﻗﻊ ﺑﺤﻴﺚ ﻻ ﺗﺆﺛﺮ ﻋﻠﻰ اﺳﺘﺨﺪام اﻟﻤﻮﻗﻊ او اﻟﻤﺮﻓﻖ‬.1
.‫ ﺿﺮورة وﺟﻮد رﻗﻢ ﻫﺎﺗﻒ اﻟﻤﺴﺌﻮل ﺑﺎﻟﺸﺮﻛﺔ )اﻟﻤﻘﺎول( اﻟﻘﺎﺋﻤﺔ ﺑﺎﻷﻋﻤﺎل ﻟﻼﺗﺼﺎل ﺑﻪ ﻋﻨﺪ اﻟﻠﺰوم وﺧﺎﺻﺔ ﻓﻲ ﺣﺎﻻت اﻟﻄﻮارئ‬.1
.‫ اﻟﺘﺰام ﻛﺎﻓﺔ اﻟﻌﺎﻣﻠﻴﻦ ﺑﺤﻤﻞ إﻗﺎﻣﺎت ﻧﻈﺎﻣﻴﺔ ﺗﺒﻌﺎ ً ﻷﻧﻈﻤﺔ اﻟﺪوﻟﺔ‬.1
‫ ﻳﺠﺐ ﺗﻘﺪﻳﻢ اﻟﺒﻄﺎﻗﺔ اﻟﻤﺪﻧﻴﺔ ﻟﻤﻬﻨﺪس أو اﻟﻤﺴﺆول ﻋﻦ ﻣﻮﻗﻊ اﻟﻌﻤﻞ اﻟﻤﻌﺘﻤﺪ ﻟﻠﺸﺮﻛﺔ )اﻟﻤﻘﺎول( ﻟﻴﻜﻮن اﻟﻤﺴﺆول اﻟﻮﺣﻴﺪ ﻋﻦ ﺗﺼﺮﻓﺎت‬.1
.‫وﺗﺠﺎوزات اﻟﻌﺎﻣﻠﻴﻦ ﻟﺪﻳﻪ‬
.‫ ﻳﻤﻨﻊ دﺧﻮل أو ﺧﺮوج أو إدﺧﺎل اﻟﺒﻀﺎﺋﻊ واﻟﻤﻌﺪات ﻋﻠﻰ ﻛﺎﻓﺔ أﻧﻮاﻋﻬﺎ إﻻ ﻣﻦ اﻟﺒﻮاﺑﺎت اﻟﺘﻲ ﻳﺘﻢ ﺗﺨﺼﻴﺼﻬﺎ ﻣﻦ ﻗﺒﻞ إدارة اﻟﻤﺮاﻓﻖ‬.1
‫ ﻳﺠﺐ ﺗﺴﻠﻴﻢ اﻟﺒﻄﺎﻗﺎت اﻟﻤﺪﻧﻴﺔ اﻟﺘﺎﺑﻌﺔ ﻟﻌﻤﺎل اﻟﺸﺮﻛﺔ )اﻟﻤﻘﺎول( ﻋﻨﺪ اﻟﺒﻮاﺑﺔ اﻟﻤﺨﺼﺼﺔ ﻣﻦ ﻗﺒﻞ إدارة اﻟﻤﺮﻓﻖ ﻓﻲ ﻏﺮﻓﺔ اﻷﻣﻦ ﻋﻨﺪ‬.1
.‫اﻟﺪﺧﻮل‬
.‫ أن ﺗﻜﻮن ﻛﺎﻓﺔ اﻟﻤﺮﻛﺒﺎت ﻣﺮﺧﺼﺔ وﻣﺴﺘﻮﻓﺎة ﻟﺠﻤﻴﻊ ﻟﻮاﺋﺢ وأﻧﻈﻤﺔ اﻻدارة اﻟﻌﺎﻣﺔ اﻟﻤﺮور اﻟﻤﻌﻤﻮل ﺑﻬﺎ‬.1
.‫ ﻋﺪم ﺗﺸﻐﻴﻞ ﻋﻤﺎﻟﺔ ﻟﻴﺴﺖ ﻋﻠﻰ ﻛﻔﺎﻟﺔ اﻟﺸﺮﻛﺔ )اﻟﻤﻘﺎول( وﻓﻲ ﺣﺎﻟﺔ اﻟﺘﻌﺎﻗﺪ ﻣﻦ اﻟﺒﺎﻃﻦ ﻳﺠﺐ ﺗﻄﺒﻴﻖ ﻫﺬه اﻟﺸﺮوط ﻋﻠﻰ ﻣﻘﺎول اﻟﺒﺎﻃﻦ‬.1
.‫ ﻳﻤﻨﻊ إﺑﻘﺎء أو ﺗﺨﺰﻳﻦ اﻟﻤﻮاد اﻟﻘﺎﺑﻠﺔ ﻟﻼﺷﺘﻌﺎل داﺧﻞ ﻣﻮﻗﻊ اﻟﻌﻤﻞ أو ﻓﻲ اﻷﻣﺎﻛﻦ اﻟﻌﺎﻣﺔ ﻟﻠﻤﺮﻓﻖ‬.1
.‫ وﺳﻴﺘﻢ إﻳﻘﺎف اﻟﺘﻐﺬﻳﺔ ﻓﻲ ﺣﺎل وﺟﻮد أﺳﻼك ﻋﺎرﻳﺔ أوﻣﻜﺸﻮﻓﺔ‬،‫ ﻳﻤﻨﻊ اﺳﺘﺨﺪام اﻟﻤﻨﺎﻓﺬ اﻟﻜﻬﺮﺑﺎﺋﻴﺔ اﻟﻤﺆﻗﺘﺔ ﺑﺪون ﻣﻨﺎﻓﺬ ﻣﻐﻠﻘﺔ‬.1
‫ ﻳﻤﻨﻊ ﻋﻠﻰ ﻋﻤﺎل اﻟﺸﺮﻛﺔ )اﻟﻤﻘﺎول( أو ﻣﻤﺜﻠﻴﻪ ﻣﻦ ﺗﻨﺎول اﻟﻄﻌﺎم و اﻟﻤﺸﺮوﺑﺎت ﻓﻲ اﻷﻣﺎﻛﻦ اﻟﻌﺎﻣﺔ ﻟﻠﻤﺮﻓﻖ أو اﻟﺘﺪﺧﻴﻦ داﺧﻞ ﺣﺪود ﻣﻮﻗﻊ‬.1
.‫اﻟﻌﻤﻞ أو ﻣﺒﺎﻧﻲ اﻟﻤﺮﻓﻖ‬
،‫ ﺗﻠﺘﺰم اﻟﺸﺮﻛﺔ )اﻟﻤﻘﺎول( ﺑﺈﺧﻄﺎر اﻟﻤﺸﺮف ﻣﻦ ﻗﺒﻞ إدارة اﻟﻤﺮﻓﻖ ﺑﺄي ﺗﺴﺒﺒﻪ أو أي ﻋﻄﻞ ﻳﻘﻊ ﻓﻲ ﺣﺪود ﻣﻨﻄﻘﺔ ﻋﻤﻠﻪ وذﻟﻚ ﻓﻮر وﻗﻮﻋﻪ‬.1
.‫وﻳﺘﻢ إﺣﻀﺎر اﻷﻣﻦ واﻟﺴﻼﻣﺔ إذا اﻗﺘﻀﻲ اﻷﻣﺮ ذﻟﻚ‬
‫ ﺗﺤﺖ إﺷﺮاف اﻟﺠﻬﺔ اﻟﻤﺸﺮﻓﺔ‬،‫ ﺗﻠﺘﺰم اﻟﺸﺮﻛﺔ )اﻟﻤﻘﺎول( اﻟﺒﺪء ﻓﻲ إﺻﻼح اﻟﻌﻄﻞ ﺑﺎﻟﻄﺮﻳﻘﺔ اﻟﺼﺤﻴﺤﺔ وﺣﺴﺐ اﻷﺻﻮل اﻟﻔﻨﻴﺔ ﻓﻮر وﻗﻮﻋﻪ‬.1
.‫ﻣﻦ ﻗﺒﻞ إدارة اﻟﻤﺮﻓﻖ ﻋﻠﻰ اﻟﺘﻨﻔﻴﺬ‬

1. It's prohibited to use temporary electrical outlets without closed outlets; and in case bare wires have been ‫ ﺗﺘﻌﻬﺪ اﻟﺸﺮﻛﺔ )اﻟﻤﻘﺎول( ﺑﺘﺼﻠﻴﺢ ﻛﺎﻓﺔ اﻷﺿﺮار اﻟﻤﺎدﻳﺔ وﻋﻠﻰ ﺣﺴﺎﺑﻬﺎ اﻟﺨﺎص ﻋﻨﺪ ﺣﺪوﺛﻬﺎ ﻣﻊ اﻟﺘﻌﻬﺪ ﻋﻠﻰ ﺳﻼﻣﺔ اﻟﻤﻈﻬﺮاﻟﻌﺎم ﻟﻤﻮﻗﻊ‬.1

public places of the facility or smoke within the workplace or the buildings of the facility. .‫ ﺗﻠﺘﺰم اﻟﺸﺮﻛﺔ )اﻟﻤﻘﺎول( ﺑﺤﺠﺰ ﺣﺎوﻳﺔ ﻻﺳﺘﺨﺪاﻣﻬﺎ ﻟﺘﺠﻤﻴﻊ اﻟﻨﻔﺎﻳﺎت وﺗﺴﻮﻳﺔ اﻟﺪﻓﻌﺔ ﻟﻠﺤﺎوﻳﺔ ﻗﺒﻞ ﺗﻨﻔﻴﺬ اﻟﻌﻤﻞ ﺑﺎﻟﺘﻨﺴﻴﻖ ﻣﻊ إدارة اﻟﻤﺮاﻓﻖ‬.1
1. The company (contractor) shall immediately notify the supervisor appointed by the Facility Management :‫ ﻳﻠﺘﺰم اﻟﻤﻘﺎول ﺑﺘﻌﻠﻴﻤﺎت اﻟﺴﻼﻣﺔ ﻣﻦ ﺣﻴﺚ‬.1
with any default or damage caused within its place of work; and security and safety personnel shall be called ‫• اﺳﺘﻌﻤﺎل اﻟﻠﻮﺣﺎت اﻹرﺷﺎدات ﻟﺘﺤﺪﻳﺪ ﻣﻨﻄﻘﺔ اﻷﻋﻤﺎل‬

1. The company (contractor) shall immediately and correctly fix a fault pursuant to applicable technical .‫• ﻣﺮاﻗﺒﺔ ﻣﻮاﻗﻊ اﻷﻋﻤﺎل ﺑﺼﻔﺔ ﻣﺴﺘﻤﺮة ﻣﻦ ﻗﺒﻞ اﻟﺠﻬﺎت اﻟﻤﺴﺌﻮﻟﺔ واﻟﺘﺄﻛﺪ ﻣﻦ ﺗﻮﻓﻴﺮ وﺳﺎﺋﻞ اﻟﺴﻼﻣﺔ ﺑﻬﺎ ﺣﺴﺐ ﺗﻌﻠﻴﻤﺎت اﻟﻤﻌﻤﻮل ﺑﻬﺎ‬

1. The company (contractor) shall fix all material damages at the expenses of the same upon occurrence .‫• اﻟﺘﺄﻛﺪ ﻣﻦ وﺿﻊ أﺷﺮﻃﺔ ﺗﺤﺬﻳﺮﻳﺔ ﻟﻸﻣﺎﻛﻦ اﻟﺨﻄﺮة‬
thereof; and shall also maintain the public appearance of both the workplace and the facility. ‫ ﻳﺤﻖ ﻹدارة اﻟﻤﺮﻓﻖ إﻳﻘﺎف اﻟﺘﻨﻔﻴﺬ واﻷﻋﻤﺎل إذا أﺧﻠﺖ اﻟﺸﺮﻛﺔ )اﻟﻤﻘﺎول( اﻟﻘﻮاﻋﺪ اﻟﻌﺎﻣﺔ أو اﻟﺘﻌﻠﻴﻤﺎت اﻟﺴﺎﺑﻘﺔ أو ﻋﻨﺪ اﻟﻈﺮوف‬.1
1. The company (contractor) shall move the waste and debris of work immediately after finishing work and .‫اﻻﺳﺘﺜﻨﺎﺋﻴﺔ‬
disposing of the same outside the borders of the facility. ‫ ﻳﺤﻖ ﻟﻜﻞ ﺟﻬﺔ ﻣﺸﺮﻓﺔ ﻋﻠﻰ اﻟﻤﺸﺮوع وﺿﻊ ﻣﻮاﺻﻔﺎت واﻟﺘﺰاﻣﺎت اﻷﻣﻦ واﻟﺴﻼﻣﺔ اﻟﺨﺎﺻﺔ ﺑﻬﺎ وﺗﺰوﻳﺪ اﻹدارة اﻟﻌﺎﻣﺔ ﻟﻸﻣﻦ واﻟﺴﻼﻣﺔ‬.1

fixed properly as per manufactures MSDS. Use of grinders to be on proper working platform, not on ladders
