● Original Certification of Good Moral from Senior High School
● Photocopy of PSA Married Certificate (if married female)

Requirements for Admission: Graduate Studies

● Recent 2” x 2” photograph with white background and collar
● Certificate of Eligibility to Transfer/Honorable Dismissal from the Previous School
● Photocopy of Grades/Transcript of Records
● Photocopy of PSA Birth Certificate (if not an AUP graduate)
● Photocopy of PSA Married Certificate (if married female)

Requirements for Transferees from other Colleges or Universities

● Recent 2” x 2” photograph with white background and collar
● Certificate of Eligibility to Transfer/Honorable Dismissal from the Previous School
● Photocopy of Grades/Transcript of Records
● Photocopy of PSA Birth Certificate
● Original Certificate of Good Moral from Previous School
