studies in Tarlac State University (University Name) with the course of Bachelor of

My child wants to participate in ”GAME OF SHARKS 2023”, and I am writing this

letter to let you know that I am allowing my child to participate in the said event
.The said screening will take place on the 15th – 20th day of January in the year of
2024. I will make sure this would not affect Chris Ian R. Dela Fuente (Student’s
Name) education.

I am fully aware that the school is not responsible for unexpected incidents that may
happen that are out of the control of the officers, staff, and the teachers. I do
understand that the school is doing the best it can for the safety of the students and
