On is used to denote
location on a street,
farm, island,
mountain or hill.
 EXAMPLE
a. Her house is on Rizal Street in Laoag City.

b. London is on the river Thames.

c. I'd love to live on the Isle of Wight.

 AT
At is used to refer to
a given address,
street, town, or city.
 EXAMPLE
a. My aunt lives at Block 12 Lot 21 Eastwood
Subdivision in Montalban, Rizal.

b. I live at 15 Craig Street.
