1) Purpose:
The purpose of this activity is to ensure that the unloading unit from towing to ground. This method
statement shall ensure that the installation is executed safely. This method statement applies to
everyone who is related to work in PT Linesia Adofa Lestari all site.

2) Scope:
This activity is carried out in the PT Linesia Adofa Lestari site .

The activity takes place in client area and this activity is medium risk. When unloading unit that
needs to be considered is the clear area. Skill and accuracy are required during unloading this unit.
Make sure area clear for any obstacle and people who not capable for this activity.

8) PPE required:
1. Safety helmet.
2. Hand gloves.
3. Work uniform
4. Safety Shoes.
5. Vest with reflector
6. Full Body Harness

9) Competency certifications:
Competency standard for driver must have driving lisence and for the operator must have SIO from
Disnaker RI. The lisence must be valid and not expired.

10) Step of Activity:

Duration of Activity : 1 Day
Description of the Activity (Step by Step)
1 Ensure that all document of this activity have been approved

2 Make sure the unit unloading area is in the clear area and there are no obstructions

3 Make sure the unloading area is flat and firm

Make sure the parking brake is engaged when unloading the unit

5 Ensure that no unauthorized persons enter the work area

6 Ensure that lifting activity in accordance with lifting plan

7 Make sure the person doing the unloading is a competent person as proven by SIO

8 Take the signal during unloading unit

9 Do this activity by the smooth

• Ensure that when do the work, please the work area is separated by a 360° barricade and only
people involved in the work are allowed to enter the work area
• Make sure the towing unit has wheel chocks installed on the wheels during the unloading process
