processed to a third party, either within or outside the Oflice, without permission from the Headof
Office.

I am fully aware that any unauthorized release or negligence in the handling of personal
information is considered a breach of my duty to maintain confidentiality, which could serve as
ground for immediate dismissal and/or possible personal liability, pursuant to Republic Act No.
10173 or the Data Privacy Act of 2012, or any legal action arising from such breach.
