COMMITMENT OF UNDERTAKING

I, NESTLIE A. LIBRADILLA, Filipino Citizen, of legal age, and a resident of PUROK 2, BYPASS
ROAD, SANTA ISABEL, DIPOLOG CITY, hereby acknowledge that I voluntarily participate in the DTI
and Google Career Certification Program 2024 and understand that the information provided relates to my
intent in availing the training under said program and that the information that will be collected, processed,
protected, shared, and retained by the DTI will be used for its pursuit of legitimate purposes, specifically for
the online training and marketing campaigns. Thus, I hereby allow the DTI - to collect, use, and share
personal data for its pursuit of legitimate interests as scholarship grant provider in accordance with RA
10173 or the Data Privacy Act of 2012.

I further manifest and declare that I will actively participate by finishing the course within the duration of
March 2024 - December 2024. I adhere to the rules and regulations of the upskilling scholarship program
and hereby undertake and commit the following:

1. Authorize the DTI to use my personal information such as my name, picture, and testimonials in
any promotional activity relating to the scholarship programs;
2. Maximize this training program and explore the courses and resources on the platform;
3. Maintain integrity by keeping my credentials confidential;
4. Keep my account ACTIVE. Should I fail to open my account within 2 weeks, my access will be
revoked and be given to waitlisted participants;
5. Comply with instructions given by the administrators;
6. Undergo the mandatory assessment as applicable;
7. Actively participate should the DTI conduct a tracer study on the participants of the said project;
8. Authorize the DTI to use my name, email address, and summary of completed courses to the DTI
ECO Partners for possible employment at the end of the training program; and
9. Utilize all the skills obtained as practicable as possible and inform the DTI of updates and results
such as employment, career advancement, etc.

This consent and authorization remain valid and subsisting for a limited period consistent with purposes or
until otherwise revoked or canceled in writing.
